苏州吴江区妇女高危人乳头状瘤病毒感染现状分析 被引量:5

Analysis of high-risk human papillomavirus infection among women in Wujiang district of Suzhou

摘要 目的调查苏州吴江区人乳头状瘤病毒(human papillomavirus,HPV)感染现状及HPV分型的分布情况,分析其与TCT和病理活检结果的相关性。方法苏州吴江区检测中心参与初筛的患者进行23种HPV基因亚型检测,HPV检测结果高危阳性的受检者转诊进行液基细胞学检查,两者均异常者或单独HPV16、HPV18阳性再行阴道镜下活检。结果5 371例标本HPV总感染率为21.09%,≤25岁和46岁及以上HPV感染率较高,HPV单一性感染和多重性感染在各年龄阶段之间差异有统计学意义(P<0.01);多重HPV感染则以≤25岁和≥51岁高发;HPV16、HPV18阳性且TCT结果阳性的患者在不同宫颈病变中差异有统计学意义(P<0.05)。结论本地区最常见的高危HPV型别为16型、18型、33型和58型,而HPV不同亚型的单一和混合感染存在明显的年龄差异,导致宫颈病变的程度有所不同。 Objective To evaluate the infection status of human paplillomavirus( HPV) and distribution of its genotypes in Wujiang district of Wuzhou,and to analyze the correlation among the HPV genotyping,TCT and biopsy results. Methods Patients participating in the primary screening of Wujiang district Examination Center were conducted for the detection of 23 kinds of HPV gene subtypes. The subjects with high risk positive HPV test results were conducted for the examination of liquid based cytology,and those with both the 2 tests abnormal or HPV16,HPV18 positive would be conducted for biopsy. Results The overall infection rate of 5 371 cases was 21. 09%. The prevalence rate of high-risk HPV( hr HPV) was high among ≤25 and 〉46 years old group. There was statistical significance on the difference of single or multiple infections and among every age stages( P〈0. 01); and the high peak age of HPV multiple infections were ≤25 and ≥51 years old. Meanwhile,the difference of patients with positive HPV16,HPV18 and positive TCT result were statistically significant in different cervical lesions( P〈0. 05). Conclusion The most popular hr HPV genotypes are HPV16,HPV18,HPV33 and HPV58,and there is obvious difference in age distribution for single and combination infections in this area,which leads to different degree of cervical lesions.
