丹红注射液治疗糖尿病肾病的临床效果及对血小板CD62p、PAC-1表达的影响 被引量:9

摘要 目的探讨丹红注射液治疗糖尿病肾病的临床效果及对血小板CD62p、PAC-1表达的影响。方法选取2014年12月~2015年12月河北北方学院附属第一医院收治的糖尿病肾病患者95例。将其随机分为观察组(50例)和对照组(45例)。对照组给予常规降糖、降压、降脂药物治疗,观察组在此基础上加用丹红注射液治疗,治疗2周,观察两组治疗前后血脂、肾功能相关指标,并检测血小板CD62p、PAC-1的表达。结果观察组总有效率为90.0%(45/50),对照组总有效率为64.4%(29/45),差异有高度统计学意义(x^2=6.905,P〈0.01)。两组治疗前血脂、尿蛋白排泄率(UAER)、尿素氮(BUN)、肌酐(SCr)、CD62p、PAC-1比较差异无统计学意义(P〉0.05)。对照组治疗前后总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL~C)和低密度脂蛋白胆固醇(LDL—C)比较差异无统计学意义(P〉0.05),观察组治疗后TC、TG、LDL—C明显下降,HDL—C明显升高,且与对照组治疗后比较差异有统计学意义(P〈0.05)。两组治疗后UAER、BUN和SCr均较治疗前明显改善,但观察组下降更显著,差异有统计学意义(P〈0.05)。观察组治疗后血小板CD62p、PAC-1阳性表达率明显低于治疗前及对照组治疗后(P〈o.05),而对照组治疗前后比较差异无统计学意义(P〉o.05)。结论丹红注射液可明显抑制糖尿病。肾病患者CD62p、PAC-1的表达,减少尿蛋白,降低血脂,改善肾功能。 Objective To investigate the clinical effect of Danhong Injection in the treatment of diabetic nephropathy and its influence on the expression of platelet CD62p, PAC-1. Methods Ninety five cases with diabetic nephropathy admitted to the First Affiliated Hospital of Hebei North University from December 2014 to December 2015 were select- ed and randomly divided into the observation group (50 cases) and the control group (45 cases). The patients in the control group were treated by conventional hypoglycemic, hypotensive and lipid-lowering drugs, while the patients in the observation group were treated by Danhong Injection on the basis of conventional therapy for 2 weeks. The blood lipid, renal function indexes of the two groups before and after treatment were observed, and the expression of platelet CD62P and PAC-1 were detected. Results The total effective rate of the observation group was 90% (45/50), the total effective rate of the controlk group was 64.4% (29/45), and the difference was highly statistically significant (~2=6.905, P 〈 0.01). There were no significant differences in blood lipid, urinary protein excretion rate (UAER), serum creatinine (SCr), CD62p, PAC-1 values between the two groups before treatment (P 〉 0.05). There were no significant differences of total cholesterol tiC), triacylglycerol (TG), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterin (LDL-C) before and after treatment in the control zrouo (P 〉 0.05). After treatment. TC. TG and LDL-C in the observation group were decreased significantly, HDL-C was increased significantly, and the changes in the observation group were more obvious than those in the control group (P 〈 0.05). UAER, BUN and SCr of the two groups after treatment were improved more sig- nificantly than those before treatment, while the observation group was decreased more significantly than those of control group, the differences were statistically significant (P 〈 0.05). The positive rate of CD62p and PAC-1 in the observation group after treatment were significantly lower than those before treatment and those of control group after treatment (P 〈 0.05), while those in the control group had no significant differences before and after treatment (P 〉 0.05). Conclusion Danhong Injection can significantly inhibit the expression of CD62p and PAC-1 in patients with diabetic nephropathy, reduce urinary protein, lower blood lipid, and improve renal function.
