1型心肾综合征相关炎性因子水平变化及临床意义 被引量:1

摘要 目的探讨1型心肾综合征(cardiorenal syndrome,CRS)患者相关性因子水平变化及其临床意义。方法回顾性分析2013年12月—2015年12月于赣榆县人民医院心内科和徐州医学院第二附属医院心内科住院确诊的心力衰竭患者120例,依据入院后有无发生急性肾损伤分为:1型CRS组(n=60)和单纯心力衰竭组(n=60),经过积极抗心力衰竭治疗后,比较两组治疗前后N-末端脑钠肽前体(N-terminal pro brain natriuretic peptide,NT-pro BNP)、血清糖类抗原125(carbohydrate antigen 125,CA125)、血清胱抑素C(cystatin C,Cys-C)、血尿素氮(blood urea nitrogen,BUN)、血清肌酐(serum creatinine,Scr)、射血分数(ejection fraction,EF)等的水平。计量资料比较采用t检验,计数资料比较采用χ~2检验,P〈0.05为差异有统计学意义。结果两组患者入院前BUN[(11.92±2.93)、(7.65±2.14)mmol/L]、Scr[(126.20±21.68)、(87.77±19.42)μmol/L]、EF[(37.26±10.45)、(49.34±10.26)%]比较,差异均有统计学意义(均P〈0.05)。与单纯心力衰竭组比较[(48.57±22.39)、(25.81±15.47)μ/ml与(1.30±0.26)、(1.08±0.17)],治疗前后1型CRS组血清CA125[(62.45±29.35)、(39.38±18.47)U/ml]、Cys-C水平[(1.71±0.23)、(1.42±0.19)mg/L]均较高,差异均有统计学意义(均P〈0.05)。结论 CA125、Cys-C可作为确诊1型CRS、评价疾病进展的生物标记物。
