甲状腺结节微创治疗现状 被引量:17

摘要 甲状腺结节的治疗方法较多,超声引导下的微创治疗以其创伤小、疗效好、并发症少、定位快速准确等优点越来越受到患者的认可。微创治疗主要包括经皮无水乙醇注射、射频消融、微波消融、激光消融和高强度聚焦超声(HIFU)治疗。良性囊性甲状腺结节首选经皮无水乙醇注射治疗。射频消融适用于功能自主性甲状腺结节、良性囊性甲状腺结节及术后复发性的甲状腺结节等。与射频消融相比,微波消融即刻消融温度更高,形态更规则,范围更广泛,还能增加大结节的完全治疗机会、降低消融后复发率,更适用于甲状腺大结节和疑为恶性的结节。激光消融主要用于治疗甲状腺良性结节。HIFU治疗适应证尚不明确,对良恶性甲状腺结节的治疗还处于起步阶段。
