我院Ⅰ类切口手术预防性应用头孢菌素调查与分析

Investigation and analysis of prophylactic use of cephalosporins in typeⅠincision operation in our hospital

目的分析我院头孢菌素预防性应用在I类切口手术中的情况及趋势。方法随机选取我院2010~2014年行I类切口手术患者1 964例。以2010年为基准,分析5年来I类切口手术患者手术部位的感染概率、抗生素应用规律,包括种类、首次用药时间及持续时间,分析头孢菌素的使用与I类切口手术感染之间的关系。结果头孢菌素应用在I类切口手术的概率明显下降,且手术感染的概率逐年下降,P〈0.05;头孢菌素应用种类也有一定的优化,多以一代或二代头孢菌素为主;首次应用抗生素的时机选择更加合理,术中、术后选择追加抗生素,且术后2d内停药的概率升高显著,P〈0.05。结论我院I类切口手术预防性应用抗菌药物在近年来发生巨大变化,使用时间、频次、种类等都逐渐符合国家规定的要求,且感染率也有明显下降。

Objective To survey the prophylactic usage of cephalosporins in patients with type ]~ incision operation in our hospital. Methods 1 964 cases of type I incision operation patients in our hospital were chosen randomly from 2010 to 2014. Setting the year of 2010 as the baseline,the infection rate, eephalosporins application rules including types,the first medication time and dura- tion time were collected and analyzed. Results The cephalosporins application decreased significantly in type I incision operation patients P〈0.05;types of cephalosporins application were the first and the second generation. Conclusion Great changes took place in our hospital in recent years in cephalosporins application in type I incision operation patients. The application time,frequen- cy, and type,etc gradually met the requirements of the State, and the infection rate has declined obviously.
