髓内固定与掌锁定钢板内固定治疗关节内桡骨远端骨折临床疗效比较

The clinical efficacy comparison of intramedullary repair device and volar plating fixation in the reconstruction of distal radius fractures

目的探讨髓内固定和掌锁定钢板内固定治疗关节内桡骨远端骨折临床疗效。方法选取2013年6月至2014年6月期间收治的57例关节内桡骨远端骨折患者,根据其治疗方法,将其分成髓内固定组和掌锁定钢板内固定组,比较两组患者的临床资料、骨折愈合时间和腕关节功能恢复情况以及术后并发症发生率。结果髓内固定组患者骨折愈合时间(4.7±1.0周)显著低于掌锁定钢板内固定组(5.9±1.3周)(P<0.05);髓内固定组患者Gartland和Werley腕关节评分优良率(93.5%)显著高于掌锁定钢板内固定组患者(76.9%)(P<0.05);髓内固定组患者的术后并发症发生率(9.7%)显著低于掌锁定钢板内固定组患者(53.8%)(P<0.05)。结论与掌锁定钢板内固定比较,髓内固定治疗关节内桡骨远端骨折骨折愈合时间更短,疗效更好,且并发症发生率更低,值得临床推广应用。

Objective To evaluate the clinical efficacy of intramedullary repair device and volar plating fixation in the reconstruction of distal radius fractures. Methods 57 cases of patients with intra - articular distal radius fractures,during June 2013 to June 2014 in our hospital, were divided the intramedullary fixation group and the palm locking plate fixation group according to the treatment method. The fracture healing time and wrist function recovery and the rate of postoperative complication of the two groups were compared. Results The differences of general information between the two groups about age,sex,fracture location,associated injuries,fractures and fracture type reasons were not statistically significant（ P 〉 0. 05）. The healing time of intramedullary fixation group（4. 7 ± 1. 0 weeks）was significantly lower than that in the palm locking plate fixation group（5. 9 ± 1. 3 weeks）（ P 〈 0. 05）. The good rate of Gartland and Werley wrist score of intramedullary fixation group（93. 5% ） was significantly higher than in the palm locking plate fixation group（76. 9% ）（ P 〈 0. 05）. The postoperative complication rate of intramedul-lary fixation group（9. 7% ）was significantly lower than that of the palm locking plate fixation group（53. 8% ）（ P 〈 0. 05）. Conclusion The clinical efficacy of intramedullary repair device in the reconstruction of distal radius fractures was significant effect and security,and the complica-tion rate was low. It was worthy of clinical application.
