眼前段生物结构参数的年龄相关性改变与性别差异研究 被引量:3

摘要 目的:比较30岁以上正常人不同年龄段眼前段生物结构参数的变化及性别差异,为闭角型青光眼的发病机制提供证据。方法选择474例眼科门诊检查正常者为研究对象,均30岁以上,其中男213例,女261例。采用眼前段光学相干断层扫描仪,获取眼前段水平图像,用中山房角分析软件进行图片分析获得眼前段结构参数[包括房角宽度(angle open distance,AOD500)、虹膜面积、前房深度及前房容积],按年龄、性别进行分组,采用 stata12.0进行数据处理及分析。结果各年龄段 AOD500、前房深度、前房容积及虹膜面积比较差异均有统计学意义(P <0.001);男性 AOD500、前房深度、前房容积及虹膜面积均显著高于女性(P <0.001)。30-55岁年龄段,AOD500变化最大,而55岁后 AOD500变化相对平缓;男性前房容积在50岁前变化相对明显,50岁后变化相对平缓,而女性前房容积变化明显且均匀。结论30以上正常人 AOD500、前房深度、前房容积均随年龄增加呈递减趋势,而虹膜面积随年龄增加呈递增趋势;女性较男性更易患闭角型青光眼。 Objective To investigate the age-related change and gender difference in ocular anterior segment parameters in persons older than 30 years,and to provide evidence for the pathogenesis of angle-closure glaucoma.Methods A total of 474 normal subjects older than 30 years (213 males and 261 females)were selected in this study.The anterior segment optical coherence tomography(AS-OCT)was performed to obtain the anterior segment images,and the Zhongshan Angle Assessment Program was used to obtain the ocular anterior segment parameters,including angle opening distance at 500 μm(AOD500),iris area,anterior chamber depth and anterior cham-ber volume.These subjects were divided into different age and gender groups.Data were analyzed using Stata12.0.Result1s There were significant differences in AOD500,iris area,anterior cham-ber depth and anterior chamber volume among different age groups(P 〈0.001).Compared with females,these ocular anterior segment parameters significantly increased in males(P 〈 0.001). The AOD500 showed sharp change in persons aged 30-55 years old and relatively gentle change in persons aged greater than 55 years.The anterior chamber volume showed sharp change in males younger than 50 years and relatively gentle change in males older than 50 years.However,anterior chamber volume changed obviously and evenly in females.Conclusion In normal persons older than 30 years,the AOD500,anterior chamber depth and anterior chamber volume decrease but the iris area increases with age.In addition,females are more likely to suffer from angle-closure glaucoma than males.
