颈动脉体瘤的影像学诊断

Imaging diagnosis of carotid body tumor

目的探讨分析颈动脉体瘤的影像学特征。方法收集2009年至2016年经上级医院手术病理证实的5例颈动脉体瘤的影像学资料,其中4例在我院行CT检查,后将原始图像上传至工作站进行MPR、VR等后处理;1例行MR检查。通过查阅文献资料,回顾性分析颈动脉体瘤的CT、MR影像特征及CT后处理技术在颈动脉体瘤中的应用价值。结果 5例颈动脉体瘤,均为单侧,男2例,女3例,年龄28~45岁,5例均位于颈总动脉分叉处,使颈内外动脉分叉角增大;4例行CT检查,病变边缘光滑,椭圆形;增强扫描明显强化,以动脉期显著,与邻近动脉密度几近相似,2例病变表面见增粗迂曲的血管影;1例行MR检查,病变较大,边界清晰,边缘见分叶,T1WI病变内见＂椒盐＂征,增强T1WI显示病变明显强化。结论颈动脉体瘤CT及MR具有特征性表现,CT后处理技术及MR多方位扫描可以清晰显示病变部位及病变与邻近血管的位置关系,从而指导临床手术。

Objective To analyze the Imaging features of carotid body tumor. Methods 5 cases of carotid body tumor imaging data by pathology were collected, 4 cases of CT examination and Volume Rendering（VR） and multi planar reconstruction（MPR）, 1case of MR examination. the imaging features and CT post-processing technique in the application value of carotid body tumor were analyzed retrospectively. Results 5 cases was all single, 2 cases were male, 3 cases were female,age 28 to 45 years old, 5 cases were all located at the bifurcation of the carotid artery, usually caused the internal and external carotid arteries shifted,and the angle between the two arteries became enlarged; 4 cases of CT examination,defined margin and marked enhancement, significantly to arterial phase, and the adjacent arteries density was almost similar, 2 cases of lesions in surface showed circuitous vessels. 1case of MR examination, lesions was bigger, well-defined and lobulated, the lesion showed ＂pepper and salt＂sign on T1 WI, marked enhancement on contrast-enhanced T1 WI. CT and MR findings of carotid body tumor were characteristic, CT post-processing technique and MR multiple scan clearly showed diseased region and the position relationship between adjacent vessels, thus to guide the clinical surgery.
