摘要
目的探讨微创经皮钢板内固定术(MIPPO)治疗胫骨干骨折的临床疗效。方法将采用MIPPO技术治疗的35例胫骨干骨折患者作为MIPPO组,传统切开复位内固定手术治疗34例胫骨骨折患者作为传统手术组,比较两组切口长度、手术时间、术中失血量、住院时间、骨折愈合时间、并发症发生情况及临床疗效。结果 MIPPO组切口长度、术中失血量、住院时间、骨折愈合时间均明显短于或少于常规手术组(P<0.05),手术时间明显长于传统手术组(P<0.05)。MIPPO组优良率为94.29%%,常规手术组优良率为88.24%;两组比较,差异无统计学意义(P>0.05)。MIPPO组无骨折延迟愈合或不愈合病例,所有患者均无切口感染发生,切口均一期愈合;传统手术组骨折延迟愈合2例,浅表感染2例,经对症处理后愈合良好。结论采用MIPPO技术治疗胫骨干骨折能获得满意的临床效果,具有创伤轻、骨折愈合时间短、并发症少等优点。
Objective To explore the clincal effects of minimally invasive percutaneous plate osteosynthesis( MIPPO) technique in the treatment of tibial shaft fracture. Methods A total of 35 patients with tibial fracture were treated by minimally invasive percutaneous plate osteosynthesis as MIPPO group;34 cases were treated by conventional open reduction and plate fixation as traditional operate group. The incision length,operative time,blood loss,hospital stay,bone healing time,complication rate and clinical effect were compared between the two groups. Results The incision length,blood loss,hospital stay and bone healing time in the MIPPO group were significantly shorter or less than that of traditional operate group. The excellent and good rate was 94. 29% in the MIPPO group,and 88. 24% in the traditional operate group,the differences were not statistically significant( P〉0. 05). There was not delayed union or nontnion in the MIPPO group,and all the incision healed with on woud infection. In the traditional operate group,2 cases of delayed union,2 case of superficial infection were observed. Conclusion MIPPO technique can achieve satisfactory functional recovery in treatment of the tibial fractures,it has the advantage of slight trauma,short time of bone union and few complocations.
