短期胰岛素强化治疗对2型糖尿病患者血糖、血脂及胰岛功能的影响

Effect of short-term intensive insulin treatment on blood glucose,blood lipid and islet function in patients with type 2 diabetes mellitus

目的探讨短期胰岛素强化治疗对初诊2型糖尿病患者血糖、血脂及胰岛功能的影响。方法 88例初诊2型糖尿病患者随机分为对照组和观察组,每组44例。对照组行常规口服降糖药治疗,观察组在对照组的基础上进行短期胰岛素治疗。比较2组治疗前、后血糖、血脂及胰岛功能变化情况。结果 2组患者空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A1c)较治疗前均显著降低,且观察组降低更显著,差异有统计学意义(P<0.05);2组患者三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)较治疗前均显著下降,高密度脂蛋白胆固醇(HDL-C)较治疗前显著升高,观察组各指标差异更显著,差异有统计学意义(P<0.05);2组患者胰岛素抵抗指数(HOMA-IR)较治疗前显著下降,胰岛素分泌指数(HOMA-β)和空腹胰岛素(FINS)均显著升高,且观察组差异更显著,差异均有统计学意义(P<0.05)。结论短期胰岛素强化治疗可明显改善患者血糖、血脂及胰岛功能。

Objective To investigate the effect of short-term intensive insulin treatment on glucose,lipid and islet function in patients with type 2 diabetes mellitus. Methods A total of 88 patients with type 2 diabetes mellitus were randomly divided into observation group（ n = 44） and control group（ n = 44）. The Patients in both groups were treated by conventional oral hypoglycemic drugs,however,the patients in observation group,on the basis of control group,were treated by short-term intensive insulin. The levels of blood glucose,blood lipid and islet function before and after treatment were detected and compared between two groups. Results After treatment,the levels of FPG,2 h PG,Hb A1 c in both groups were obviously decreased,especially in observation group（ P 〈0. 05）. After treatment,the levels of TG,TC,LDL-C in both groups were obviously decreased,however,the levels of HDL-C were obviously increased,as compared with those before treatment,moreover,the differences in observation group were more significant than those in control group（ P 〈0. 05）. After treatment,the HOMA-IR in both groups was obviously decreased,however,HOMA-β and FINS were obviously increased,as compared with those before treatment,especially in observation group（ P 〈0. 05）. Conclusion The short-term intensive insulin treatment can obviously improve blood glucose,blood lipid and islet function of patients with type 2 diabetes mellitus.
