右心室主动电极起搏与被动电极起搏间起搏参数在心脏病中的差异

Parameter differences between right ventricle active electrode pacing and passive electrode pacing in heart diseases

目的分析右心室主动电极起搏与被动电极起搏间起搏参数在心脏病中的差异。方法随机选取宁波市医疗中心李惠利医院2011年5月～2016年2月收治的120例起搏器植入患者，依据右心室起搏方法将患者分为主动电极起搏组（n=60）和被动电极起搏组（n=60），对两组患者的心室起搏参数及并发症发生情况进行统计分析。结果主动电极起搏组患者的心室阻抗、感知均显著低于被动电极起搏组（P〈0．05），但两组患者的心室P波幅度之间的差异无统计学意义（P〉O．05），两组患者心室阈值之间的差异无统计学意义（P〉0．05）；主动电极起搏组患者的并发症发生率1．7％（1／60）显著低于被动电极起搏组10．0％（6／60）（P〈O．05）。结论右心室主动电极起搏在心脏病中的阻抗、感知均低于被动电极起搏间起搏，但二者的阈值之间的差异不显著，前者较后者具有较低的并发症发生率，更为安全可行，值得在临床推广。

Objective To analyze the parameter differences between right ventricle active electrode pacing and passive electrode pacing in heart diseases. Methods 120 patients with pacemaker implantation treated in our hospital from May 2011 to February 2016 were selected and divided according to the pacing pattern of right ventricle into active electrode pacing group （n=60） and passive electrode pacing group （n=60）. The pacing parameters of ventricle, and incidence of complications in both groups were analyzed. Results The impedance and perception of ventricle were significantly lower in the active electrode pacing group than in the passive electrode pacing group（P〈0.05）. There was no significant differ- ence in ventricular P-wave amplitude between two groups （P〉0.05）. There were no significant differences in threshold values of ventricle between two groups（P〉0.05）. The incidence rate of complications was 1.7%（1/60） in the active elec- trode pacing group, which was significantly lower than the 10.0% （6/60） in the passive electrode pacing group （P〈0.05）. Conclusion The impedance and perception of right ventricle active electrode pacing are significantly lower than those in the passive electrode pacing, but the difference in threshold value is not significant between two groups. Compared with passive electrode pacing, active electrode pacing has lower incidence rate of complications and favorable safety, thus is worthy to be promoted.
