食管癌7.0-T MRI成像及其数字切片图像特点 被引量:2

摘要 目的利用7.0-T MRI显示正常食管壁的结构及食管癌的浸润范围,同时利用数字切片进行对比。方法手术切除的食管癌组织提取10例标本,包括癌组织、交界及正常食管组织,大小约5 cm×1.5 cm,甲醛固定后进行7.0-T MRI成像;然后切片染色进行数字切片扫描。结果 7.0-T MRI成像T2WI序列中,黏膜上皮层为稍低信号、黏膜固有层为略高信号、黏膜肌层为低信号、黏膜下层为高信号、肌层的内层环形肌和外层纵行肌为低信号、环形肌和纵行肌间联合组织为高信号、外膜为高信号。正常食管壁大致分成8层,且组织各层间分界较清晰。这些与数字切片图像大致相对应。在T1WI中,黏膜层为稍低信号、黏膜肌层为低信号、黏膜下层为低信号、肌层低信号、外膜为略高信号,各层之间信号差异较小,各层之间分界较差。而食管癌组织中,由于肿瘤组织的浸润,食管壁各层之间界限消失,T1WI及T2WI中肿瘤组织大部分表现于低信号。结论 7.0-T MRI体外成像,能较清晰地显示正常食管壁的结构,并能显示肿瘤组织在食管壁的浸润及破坏情况。
