脑外伤后患者下丘脑—垂体—肾上腺轴功能障碍对预后的影响被引量：7

摘要目的分析脑外伤后患者下丘脑-垂体-肾上腺轴(HPA轴)功能障碍对创伤性脑损伤(TBI)预后的影响。方法选择48例TBI患者作为观察对象,其中轻型10例,中型13例,重型18例,特重型7例。于TBI后第3天早上8点采集患者静脉血2 mL,对血清皮质醇水平进行测定,并口服地塞米松进行治疗;TBI之后第4天早上8点采集2 mL静脉血检测。对患者的住院时间、格拉斯哥预后评分(GOS)等进行观察对比。结果 HPA轴功能障碍发生率最高的为特重型患者,其次为重型患者、中型患者和轻型患者,差异有统计学意义(P<0.05);而HPA功能良好的患者住院时间及KPS评分均优于HPA轴功能障碍的患者,差异有统计学意义(P<0.05)。结论TBI患者的HPA轴功能与患者预后之间存在一定的关系,HPA轴功能越差,患者的预后则越差。
