老年肺部感染合并心功能不全患者硝酸甘油微泵治疗的安全性与有效性评估

Safety and efficacy of nitroglycerin micro pump for elderly patients with lung infections combined heart failure

目的分析老年肺部感染合并心功能不全患者行硝酸甘油微泵的安全性与有效性,为临床治疗提供参考。方法选取医院2014年2月-2015年4月收治老年肺部感染合并心功能不全患者56例为研究对象,随机为研究组和对照组各28例,对照组患者采取吸氧、控制感染、利尿等治疗,研究组患者在此基础上采取硝酸甘油微泵治疗,分析两组患者治疗效果。结果研究组患者治疗总有效率91.1%,显著高于对照组75.0%(P<0.05),治疗前,两组患者PaO_2/FiO_2和CVP差异无统计学意义,治疗后研究组PaO_2/FiO_2(269.34±41.28)显著高于对照组(208.16±49.67)(P<0.05),研究组CVP(8.92±2.54)cmH_2O显著低于对照组(11.06±2.25)cmH_2O(P<0.05);研究组住院时间、肺部啰音减少时间等显著短于对照组(P<0.05),研究组BNP恢复正常时间(2.89±1.28)d显著少于对照组(5.23±1.49)d(P<0.05);研究组1例出现头晕症状,调整硝酸甘油剂量后好转。结论老年肺部感染并心功能不全患者治疗中,采用硝酸甘油微泵治疗能够提高治疗效果,不良反应小,具有推广价值。

OBJECTIVE To analysize the safety and efficacy of nitroglycerin micro pump for elderly patients with lung infections combined heart failure,so as to provide reference for clinical treatment.METHODS A total of 56 patients with elderly lung infections combined heart failure in hospital from Feb.2014 to Apr.2015 were selected,and patients were divided into study group and control group according to digital randomly,28 cases in each group.Control group patients were taken treatment of oxygen uptake,infection control,diuretic,while study group patients were taken treatment of nitroglycerin micro pump,and the effect of patients were compared.RESULTS The total efficiency（91.1%）in study group was significantly higher than control group（75.0%）（P〈0.05）.Before the treatment,PaO2/FiO2 and CVP were no significant difference in two groups of patients.After treatment,the PaO2/FiO2（269.34 ± 41.28）in study group was significantly higher than that in control group（208.16 ± 49.67）（P〈0.05）,the CVP（8.92 ± 2.54）cmH2O in study group was significantly lower than that in control group（11.06±2.25）cmH2O（P〈0.05）.Hospital stay,pulmonary rales reduceing time in study groups were significantly shorter than those in control group（P〈0.05）,the study group BNP recovery time（2.89±1.28）d was significantly less than that in control group（5.23±1.49）d（P〈0.05）.There was 1patient with dizziness symptoms in study group,but improved after adjusting the dose of nitroglycerin.CONCLUSIONIn the treatment on elderly with lung infection combined heart failure,nitroglycerin micro pump therapy can improve the therapeutic effect with small adverse reactions,which has promotional value.
