联合一体化护理对剖宫产产妇角色转换的促进作用

Promoting effect of " maternal physiology and psychological integration" combined with "patient and family integration" on maternal role change after cesarean section

目的探讨"产妇生理与心理一体化"联合"患者与家庭一体化"对剖宫产产妇角色转换的促进作用。方法选择2014年5月-2015年6月在无锡市人民医院行剖宫产的产妇186例,随机分成对照组和研究组,每组各93例。对照组采用常规护理,研究组采用常规护理+产科一体化护理,比较两组焦虑、抑郁情况,产后泌乳量,血清催乳素(PRL)水平,角色转换及新生儿照护能力评分。结果护理后,研究组焦虑、抑郁评分均低于对照组,差异均有统计学意义(P<0.05);研究组角色转换及新生儿照护能力评分均高于对照组,差异均有统计学意义(P<0.05);研究组泌乳量及血清PRL水平均高于对照组,差异均有统计学意义(P<0.05)。结论对剖宫产产妇实施"产妇生理与心理一体化"联合"患者与家庭一体化"护理能有效改善产妇的焦虑、抑郁情绪,促进产妇角色转换,可推广应用。

Objective To explore the promoting effect of ＂ maternal physiology and psychological integration＂ combined with ＂ patient and family integration＂（ obstetric integration nursing） on maternal role change after cesarean section.Methods A total of 186 pregnant women undergoing cesarean section in the hospital from May 2014 to June 2015 were selected and randomly divided into control group and study group,93 cases in each group.The cases in control group received routine nursing,while the cases in study group received routine nursing combined with obstetric integration nursing.Anxiety and depression scores,postpartum lactation amounts,serum lactation levels,role change and neonatal care ability scores in the two groups were compared.Results After nursing,anxiety and depression scores in study group were statistically significantly lower than those in control group（ P〈0.05）;role change and neonatal care ability scores in study group were statistically significantly higher than those in control group（ P〈0.05）;postpartum lactation amount and serum lactation level in study group were statistically significantly higher than those in control group（ P 〈 0.05）.Conclusion ＂ Maternal physiology and psychological integration＂ combined with ＂ patient and family integration＂ can effectively improve maternal anxiety and depression and promote maternal role change after cesarean section,which is worthy of clinical promotion and application.
