益肺健脾汤联合空肠内营养支持在慢性阻塞性肺疾病急性加重期合并营养不良中的临床作用

The Clinical Effect of Yifei Jianpi Decoction(益肺健脾汤) Combined with Jejunal Nutrition Support in Acute Exacerbation of Chronic Obstructive Pulmonary Disease Combined Malnutrition

目的:探讨中药联合空肠内营养支持在慢性阻塞性肺疾病急性加重期(AECOPD)合并营养不良中的临床作用。方法:将94例AECOPD合并营养不良患者随机分为治疗组51例和对照组43例;治疗组给予益肺健脾汤联合空肠内营养支持,对照组给予单纯饮食支持。观察比较两组临床疗效及并发症情况。结果:治疗组和对照组治疗后的FVC、FEV1/FVC、MIP、MEP均高于治疗前(P<0.05);治疗后MIP、MEP治疗组均高于对照组(P<0.05)。治疗组治疗后的ALB、PA、TFN均高于治疗前(P<0.05),对照组治疗后的ALB、TFN均高于治疗前(P<0.05);治疗后治疗组ALB、PA、TFN均高于对照组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:在治疗AECOPD合并营养不良中,益肺健脾汤联合空肠内营养支持具有更好的疗效,并发症发生率较低。

Objective： To learn the clinical effect of Yifei Jianpi Decoction combined with jejunal nutrition support in acute exacerbation of chronic obstructive,pulmonary disease （AECOPD） combined malnutrition. Meth- ods： The 94 cases with AECOPD combined malnutrition were randomly divided into treatment and control groups, respectively 51 and 43 cases in each. Patients were given Jianpi Yifei decoction combined with jejunal nutrition support in treatment group and simple diet support in control group. The clinical symptom and com- plications were observed in the two groups. Results： The FVC, FEV1/FVC, MIP and MEP increased in two groups after the treatment （p〈0.05）, and the treatment group showed higher level of MIP and MEP than the control group （P〈0.05）. The ALB, PA and TFN were increased in treatment group after the treatment （p〈0.05）, and the ALB and TFN were increased in control group after the treatment （p〈0.05）. the treatment group showed higher ALB, PA and TFN than the control group （p〈0.05）. The difference on incidence of complications was not statistically significant （P〉0.05）. Conclusions： Yifei Jianpi decoction combined with jejunal nutrition support showed good efficacy in the treatment of AECOPD patients combined malnutrition, with low incidence of compli- cations.
