神经性厌食女学生三维人格特征与防御方式的关系研究

Relation Between Tridimensional Personality Traits and Defensive Style Among Female Students With Anorexia Nervosa

目的探讨神经性厌食(AN)女学生三维人格特征与防御方式的关系。方法选取2013年3月—2014年12月在温州医科大学附属第一医院精神卫生科门诊或住院治疗的AN患者36例为AN组,及同期温州市第八中学和温州医科大学的健康女学生36例为对照组,年龄和受教育年限与AN组匹配。采用自编一般情况调查表了解受试者的基本情况,采用三维人格问卷(TPQ)、防御方式问卷(DSQ)对受试者进行测评,比较两组TPQ、DSQ得分并进行Pearson相关分析。结果 AN组TPQ躲避伤害性(HA)维度中预期忧虑和悲观主义-不受拘束的乐观主义(HA1)〔(5.0±1.3)分比(4.2±1.1)分〕、害怕不确定-喜欢不确定(HA2)〔(4.9±1.2)分比(3.6±0.9)分〕、易疲劳/虚弱无力-有精神力(HA4)〔(5.5±1.4)分比(3.7±1.0)分〕因子得分及总分〔(19.6±3.0)分比(15.4±2.2)分〕均高于对照组,差异有统计学意义(P<0.05)。AN组DSQ不成熟防御机制总均分高于对照组,差异有统计学意义〔(4.4±1.0)分比(3.6±0.8)分,P<0.05〕。HA维度得分与不成熟防御机制维度得分及病程均呈线性正相关(r值分别为0.477、0.525,P<0.05),与BMI呈线性负相关(r=-0.367,P<0.05)。结论 AN女学生有特异性三维人格特征,这可能与其防御方式和病情有一定关联。

Objective To explore the relation between tridimensional personality traits and defensive style among female students with anorexia nervosa. Methods From March 2013 to December 2014, we enrolled 36 AN inpatients or outpatients from the Department of Mental Health of the First Affiliated Hospital of Wenzhou Medical University as AN group. We also enrolled 36 healthy female students from the Eighth Middle School and Wenzhou Medical University in the same period as control group with matched age and years of education with AN group. A self - designed questionnaire was used to investigate the basic information of the subjects, and tridimensional personality questionnarie （TPQ） and defensive style questionnaire （DSQ） were adopted to conduct tests on the subjects. Comparison was made in the scores of TPQ and DSQ between the two groups, and Pearson correlation analysis was performed. Results AN group was higher than control group in HA1 [ （5.0 ± 1.3 ） vs. （4. 2 ±1.1）], HA2 [ （4.9±1.2） vs. （3.6±0.9）J andHA4 （（5.5±1.4） vs. （3.7±1.0）] scores in the HA dimension and the total score [ （ 19. 6 ± 3.0） vs. （ 15.4± 2. 2） ] of TPQ （ P 〈 0. 05 ） . AN group was higher than control group in the total mean score of the immature defense mechanism of DSQ [ （4.4 ±1.0） vs. （3.6 ±0.8）, P〈0.05] .The score of HA dimension had positive linear correlation with the score of immature defense mechanism and the course of disease （ r values were 0.477 and 0. 525 respectively, P 〈 0. 05 ） , and had negative linear correlation with BMI （ r = - 0. 367, P 〈 0. 05 ）. Conclusion Female students with AN have tridimensional personality traits which may have correlation with its defensive style and disease condition.
