双极射频微创联合小针刀治疗腰椎间盘突出症的临床观察

Clinical observation of bipolar radiofrequency minimally invasive therapy combined with small needleknife in the treatment of lumbar disc herniation

目的观察双极射频微创介入联合小针刀治疗腰椎间盘突出症(LDH)的临床效果。方法 153例LDH患者,随机分为A、B、C三组,各51例,分别给予射频靶点热凝治疗、小针刀治疗、小针刀联合射频靶点热凝治疗,评价并比较三组患者的治疗结果。结果三组患者术后各时间段的视觉模拟评分法(VAS)评分均较术前明显降低(P<0.05),且C组均显著低于A、B组(P<0.05);治疗后C组的治疗优良率显著高于A、B组(P<0.05)。结论在LDH的治疗中,与单纯应用小针刀或射频靶点热凝治疗相比,二者联合治疗可明显提高疗效,优于椎管内外单纯治疗,值得推广。

Objective To observe clinical effect by bipolar radiofrequency minimally invasive intervention combined with small needle-knife in the treatment of lumbar disc herniation （LDH）. Methods A total of 153 LDH patients were randomly divided into groups A, B and C, with 51 cases in each group. Tlgjee groups received respectively radiofrequency target thermosetting, small needle-knife and small needle-knife combined with radiofrequency target thermosettingfor treatment. Curative effects were evaluated and compared in the three groups. Results All three groups had obviously lower visual analogue scale （VAS） scores in each postoperative time points than those before operation （P〈O.05）, and group C had much lower scores than groups A and B （P〈O.05）. After treatment, group C had obviously higher good rate than groups A and B （P〈0.05）. Conclusion Comparing with single therapy by small needle-knife or radiofrequency target thermosetting in treating LDH, their combination can remarkably improve curative effect, and it is better than single therapy and is worth promoting.
