广东省816例人工耳蜗植入术候选者情况分析

An Analysis of 816 Candidates for Cochlear Implantation in Guangdong Province

目的：了解广东省人工耳蜗救助项目受助者初筛情况及存在的问题，为完善项目管理提供依据。方法对2009～2011年广东省816例人工耳蜗候选者进行一般筛选，按国家项目制定的初筛内容及标准严格执行听力学综合检查、听觉能力、言语能力评估以及精神、智力、行为等全面筛查，分析整理筛查结果。结果2009～2011年，广东省人工耳蜗救助项目一般筛选通过率分别为76.85%、79.01%及100%，通过率逐年上升；初筛环节通过率分别为46.85%、65.56%和62.00%，暂缓例数中听觉能力、语言能力、学习能力未达标比例较大，占暂缓总人数的79.39%；不通过例数中残余听力较好比例共45例，占不通过人数的54.22%；初筛后进行审核，国家项目审核通过率分别为77.50%、94.79%及84.28%，其他项目审核通过率分别为77.78%、100%及80.00%；2009～2011年由医院复筛获得国家项目植入人工耳蜗的复筛通过率为93.55%、100%、89.83%；省其他项目复筛通过率均为100%。结论人工耳蜗资助项目通过一般筛选、初筛、讨论、公示、审核、复筛、手术、安置等程序已形成完善的筛查链和资助机制，通过行之有效的机制保证筛查质量，提高了初筛通过率，取得了良好社会效益。

Objective To review the preliminary screening data of candidates for cochlear implantation in Guangdong province and to analyze the existing problems, in order to improve the program management.Methods From 2009 to 2011, 816 candidates for cochlear implantations received the preliminary screening according to the standards of the national assistance program cluding audiological measurements, hearing and speech. Ability evaluation, and mental,intelligence and behavioral examination.Results From 2009 to 2011, the pass rates of cochlear assistance program were 76.85%,79.01% and 100% respectively, showing an obvious increase. The pass rates of preliminary screening were 46.85%,65.56% and 62.00%,respectively. Among those who failed the preliminary screening, 104 did not reach the standards in auditory, speech and learning test, accounting for 79.39%. In the failed cases, 45 had good residual hearing, accounting for 54.22%. After the preliminary screening, the data were submitted to the experts for further examination.Conclusion The pass rates of secondary screening in hospital for national assistance program were 93.55%,100% and 89.83% in 2009,2010 and 2011 respectively and those for the other programs were all 100%. Conclusion The cochlear implant assistance program has formed a complete screening procedure and funding mechanisms, which ensures the screening quality, improves the pass rate of preliminary screening and achieves good social benefits.
