全关节镜下清创减压术和直接修复术治疗巨大肩袖撕裂的效果比较 被引量:5

摘要 目的探讨全关节镜下清创减压术和直接修复术治疗巨大肩袖撕裂的疗效分析。方法回顾性分析2007年12月—2012年11月确诊巨大肩袖撕裂患者36例。采用全关节镜下直接修复术(铆钉止点重建术)19例,行全关节镜下清创减压术17例。观察两组术后情况、关节活动度、恢复运动时间及手术前后肩关节评分(UCLA)和疼痛视觉模拟评分(VAS)。结果所有病例获随访,平均11.47个月。两组术后关节活动度均较术前改善,直接修复组前屈、后伸角度小于清创减压组,外旋角度大于清创减压组,恢复运动时间短于清创减压组(P<0.05)。两组术后UCLA和VAS较术前明显改善(P<0.05)。清创减压组满意度47.0%,直接修复组满意度86.4%。结论全关节镜下直接修复术治疗巨大肩袖撕裂安全有效,清创减压术可依患者情况选用。
