老年胃癌患者心理应激水平与免疫功能的相关性

Relationship between psychological stress and immune function in elderly patients with gastric cancer

目的研究老年胃癌患者心理应激水平以及免疫功能现状,并探讨老年胃癌患者心理应激水平与免疫功能的相关性,以期为改善胃癌患者生存质量提供理论依据。方法选择恩施州民族医院2014年7月—2015年7月收治的279例老年胃癌患者为研究对象,通过症状自评量表(SCL-90)以及免疫功能相关指标(白细胞、自然杀伤细胞以及T细胞亚群)对研究者进行评价。结果处于临床Ⅰ、Ⅱ、Ⅲ、Ⅳ期的老年胃癌患者SCL-90总均分分别为(1.85±0.47)、(2.01±0.54)、(2.36±0.66)、(2.40±0.71)分,得分差异有统计学意义(P<0.05);不同临床分期的患者白细胞数、自然杀伤细胞以及T细胞亚群随分期级数增高而明显降低,差异有统计学意义(P<0.05);SCL-90各维度得分与白细胞数、自然杀伤细胞数以及T细胞亚群数(包括CD3+,CD4+,CD8+,CD4+/CD8+)检测结果呈负相关(P<0.05)。结论老年胃癌患者易出现负性心理应激,从而易导致机体的细胞免疫功能紊乱而加重胃癌患者的病情。

[Objective]To study the status of psychological stress and immune function in elderly patients with gastric cancer,explore the correlation between them,and provide a theoretical basis for improving the quality of life in patients with gastric cancer.[Methods]279 elderly patients with gastric cancer in National Hospital of Enshi Prefecture from July 2014 to July 2015 were recruited in the study. The Symptom checklist（SCL-90）and immune function indicators（including white blood cells,natural killer cells and T cell subsets）were used to evaluate the investigated subjects. [Results]The scores of SCL-0 in elderly patients with gastric cancer in clinical Ⅰ,Ⅱ,Ⅲ and Ⅳ stage were respectively（1.85±0.47）,（2.01±0.54）,（2.36±0.66）and（2.40±0.71）,and the difference was statistically significant（P〈0.05）.The levels of white blood cells,natural killer cells and T cell subsets decreased with the increasing of clinical stage,and the difference was statistically significant（P〈0.05）.The scores of all SCL-90 dimensions were negatively correlated with the detection results of white blood cells,natural killer cells and T cell subsets（including CD3＋,CD4＋,CD8＋ and CD4＋/CD8＋）（P〈0.05）. [Conclusion]Elderly patients with gastric cancer are prone to occur the negative psychological stress,which could easily lead to immune dysfunction and result in worsen the situation.
