摘要
目的研究老年2型糖尿病(type 2 diabetes mellitus,T2DM)患者体内血清25(OH)D_3水平与胰岛β细胞功能、胰岛素抵抗之间的关系。方法详细收集60例老年T2DM患者和60例健康对照者的临床资料。测定老年T2DM患者及同期健康体检者的空腹血清25-羟维生素D_3水平;老年T2DM患者空腹血糖(FPG)、空腹胰岛素(FINS)水平。使用Pearson相关分析评价血清25-羟维生素D_3水平与胰岛素抵抗、胰岛β细胞功能的相关性。采用多元逐步回归分析评价胰岛β细胞功能的影响因素。结果 1老年T2DM患者血清25(OH)D_3水平显著低于对照组[(35.32±15.24)nmol/L vs.(43.17±17.15)nmol/L,P<0.01]。2老年T2DM患者血清25(OH)D_3与HOMA2-IR、HOMA2-%S无显著相关性(r=-0.235,P=0.073;r=0.223,P=0.090),与HOMA2-%B呈显著正相关(r=0.400,P<0.01)。多元逐步回归分析显示HOMA2-%B的重要相关影响因素是25(OH)D_3(β=0.304,P<0.01)和FPG(β=-0.607,P<0.01)。结论老年T2DM患者体内血清25(OH)D_3的水平明显下降,血清25(OH)D_3水平过低可能会加重胰岛β细胞功能衰竭。
Objective To study the relationship between serum 25(OH)D_3 level of elderly type 2 diabetes mellitus( T2DM) patients and pancreatic β cell function and insulin resistance. Methods Clinical data of 60 cases of elderly T2 DM patients and 60 cases of healthy controls were collected in detail. Fasting serum 25(OH)D_3 level of elderly T2 DM patients and contemporaneous health controls were measured; And so did the fasting plasma glucose( FPG),fasting insulin( FINS) level of elderly T2 DM patients. The correlation of serum 25(OH)D_3 level and insulin resistance and pancreatic β cell function was evaluated by using Pearson correlation analysis. The influence factors of pancreatic β cell function were evaluated by using the multiple stepwise regression analysis. Results 1 The serum 25(OH)D_3 level of elderly T2 DM patients was significantly lower than the control group [( 35. 32±15. 24) nmol / L vs( 43. 17±17. 15) nmol / L,P0. 01). 2The serum 25(OH)D_3 of elderly T2 DM patients was in no significant correlation with HOMA2-IR and HOMA2-% S( r =-0. 235,P = 0. 073; r = 0. 223,P = 0. 090),and in significantly positive correlation with HOMA2-% B( r = 0. 400,P〈0. 01). Multiple stepwise regression analysis showed that 25(OH)D_3( β =0. 304,P0. 01) and FPG( β =-0. 607,P0. 01) were important related influence factors of HOMA2-% B. Conclusion Serum 25(OH)D_3 level of elderly T2 DM patients declined obviously and low serum 25(OH)D_3 level may exacerbate pancreatic β cell function function failure.
