摘要
目的:了解口腔扁平苔藓(OLP)患者外周血单个核细胞(PBMCs)中PD-1 mRNA、PD-L1 mRNA的表达及意义。方法:分别采用流式细胞术和散射比浊法分析36例OLP患者外周血淋巴细胞亚群(CD3^+、CD4^+、CD8^+、CD19^+、CD16^++56^+)状况和检测患者体液免疫指标(1gG、1gA、1gM、C3、C4)。采用荧光定量PCR法(qPCR)检测36例OLP患者及18例健康对照组PBMCs中PD-1 mRNA、PD-L1 mRNA的表达情况并分析其与OLP患者免疫功能状况的相关性。结果:OLP患者外周血中CD3^+、CD4^+、CD8^+、CD16^++56^+(NK)均低于正常值(P<0.05),CD19^+(B)高于正常值(P<0.05);血清补体C3、C4较正常值降低(P<0.05),免疫球蛋白1gM较正常值增高(P<0.05)。OLP患者PBMCs中PD-1 mRNA及PD-L1 mRNA的相对表达量均高于对照组(P<0.05)且PD-1 mRNA与PD-L1 mRNA的相对表达量呈正相关。PD-1 mRNA的相对表达量与OLP患者CD4^+呈负相关,与1gG呈正相关;PD-L1 mRNA的相对表达量与CD19^+呈正相关。结论:OLP患者PBMCs中PD-1、PD-L1 mRNA的表达上调与OLP患者免疫功能状况相关,提示PD-1/PD-L1通路可能参与OLP的免疫发病机制。
Objective:To explore the expression of PD-1 mRNA and PD-L1 mRNA in peripheral blood mononuclear cells(PBMCs) of OLP patients.Methods:The peripheral blood lymphocyte subsets(CD3^+,CD4^+,CD8^+,CD19^+,CD16^+ + 56^+) and humoral immunity indexes(1gG,1gA,1gM,C3,C4) were detected in 36 patients with OLP by flow cytometry and nephelometry respectively.The expressions of PD-1 mRNA and PD-L1 mRNA in PBMCs of 36 patients with OLP and 18 healthy individuals(the controls) were detected by fluorescence quantitative PCR,the correlation of PD-1 mRNA or PD-L1 mRNA with the immune function of OLP patients was analysed.Results:In OLP patients the percentages of peripheral blood lymphocyte subsets(CD3^+,CD4^+,CD8^+,CD19^+,CD16^+ +56^+) as well as the complement of C3 and C4 were lower(P 0.05),the percentages of CD19^+(B) and the level of lgM were higher(P 0.05),the expression levels of PD-1 mRNA and PD-L1 mRNA in rPBMCs were higher(P 0.05) than in the controls.There was positive correlation between PD-1 mRNA and PD-L1 mRNA expression level.The expression level of PD-1 mRNA was negatively correlated with CD4^+,but positively with lgG,the expression level of PD-L1 mRNA was positively correlated with CD19^+.Conclusion:The up-regulation of PD-1 mRNA and PD-L1 mRNA in PBMCs is correlated with the immunity of OLP patients.PD-1/PD-L1 pathway may play an important role in immune pathogenesis of OLP.
