乳腺彩超、钼靶及MRI在乳腺癌诊断中的临床应用 被引量:12

摘要 目的比较乳腺彩超、钼靶X线及磁共振成像(MRI)在乳腺癌诊断中的应用及优缺点。方法 54例临床疑为乳腺癌患者均行乳腺彩超、钼靶X线及MRI检查,并与病理学对照,对三种诊断技术的应用价值做出评价,并分析优缺点。结果病理学证实为乳腺癌者48例(52个病灶),良性肿瘤者6例(9个)。灵敏度:MRI(90.4%)>钼靶(82.7%)>彩超(75.0%);特异性:钼靶(55.6%)>彩超(44.4%)>MRI(33.3%)。结论在三种诊断技术中,MRI费用高,操作复杂,诊断时间长,对钙化的显示比不上钼靶;而钼靶对于致密型乳腺癌患者显示效果较差,应结合彩超或MRI检查;彩超检查便捷,无痛苦,灵活性较高,但对于无肿块者或微小钙化的乳腺癌仍有局限性。 Objective To study the clinical application of color Doppler ultrasonography, mammography and magnetic resonance imaging(MRI)on early breast cancer imaging diagnosis and compare their advantages and disadvantages.Methods 54 cases with clinical suspicion of breast cancer were checked by color Doppler ultrasonography, mammography and MRI, and were confirmed by pathology comparison. The application value of three kinds of diagnostic technology were evaluated, and the advantages and disadvantages were analyzed. Results 48 cases of breast cancer were confirmed by pathological examination, in total of 52 lesions, 6 cases of benign tumor(9 lesions). Sensitivity: MRI(90.4%)mammography(82.7%)ultrasound(75.0%); specificity: mammography(55.6%) ultrasound(44.4%) MRI(33.3%). Conclusion In three kinds of diagnostic technology, MRI is high cost, complex operation, long diagnosis time, and calcification of display is no match for mammography; The effect of mammography targets in patients with dense breast cancer is poor, should combined with color Doppler ultrasound and MRI examination; Color Doppler ultrasound is convenient,no pain, high flexibility, but for no lumps or micro calcification of breast cancer has limits.
