益元解毒法对老年骨髓增生异常综合征的疗效研究

Study on Effect of Tonifying the Origin and Detoxification Method for Myelodysplastic Syndrome in the Elderly

目的 :探讨益元解毒汤对老年骨髓增生异常综合征(MDS)的临床疗效。方法 :采用随机对照临床试验方法 ,将64例患者按随机数字表法分为两组。两组均采用支持治疗,对照组30例,予康力龙(吡唑甲基睾丸素)每次2 mg,每日3次,环孢素A3 mg/(kg·d),早晚分服。治疗组34例,康力龙每次2 mg,每日3次,环孢素A每次25 mg,每日3次,每日服用益元解毒汤剂一次。3个月为1个疗程,连续治疗2个疗程,比较2组疗效。结果 :治疗组总有效率64.71%,对照组总有效率56.66%,两组疗效相比无统计学意义(P>0.05);治疗后两组患者血常规、骨髓细胞学检查结果与治疗前相比有所改善,两组治疗后比较无统计学意义(P>0.05);两组不良反应比较,治疗组明显低于对照组(P<0.05)。结论 :益元解毒法联合小剂量环孢素A治疗老年MDS的临床疗效与常规剂量环孢素相比作用相当,且不良反应明显减少。

Objective：To explore the clinical efficacy of using tonifying the origin and detoxication method to treat myelodysplastic syndrome （MDS） in the elderly. Methods ：The method of randomized controlled clinical trials was applied,and 64 patients were divided into two groups according to random number table. In the control group with 30 cases,2 mg of stanozolol tablets was administered orally 3 times per day,and ci- closporin was administered,3 mg/kg,twice daily. In the trdatment group with 34 cases,2 mg of stanozolol tablets was administered 3 times per day,25 mg of ciclosporin A was administered,twice daily,and decoction for tonifying origin and detoxification was administered once per day. A therapeutic course is of 3 months, and the treatment lasted for 2 courses. Results：The total effective rate in the treatment group was 64.71%, and in the control group was 56.66%,with no statistical difference between the two groups（P〉0,05）. After the treatment,the hemogram and myelogram were improved compared with before the treatment,but with no statistical difference between the two groups（P〉0.05）. However,on the adverse reaction,the treatment group was significantly milder than the control group（P〈0.05）. Conclusion：In the treatment for the MDS in the elderly,the clinical efficacy of tonifying the origin and detoxification in combination with small dose of ci- closporin A was similar to routine does of ciclosporin, and the adverse reaction was significantly reduced.
