CT及MRI对良恶性胆道梗阻诊断的价值研究

Diagnosed value of CT and MRI to benign and malignant biliary obstruction

目的探讨CT、MRI对良恶性胆道梗阻诊断的价值,为提高临床对良恶性胆道梗阻诊断准确率提供理论依据。方法回顾性分析2012年5月—2013年12月河南省直第三人民医院收治的胆道梗阻患者135例,其中良性48例,恶性87例,有53例患者采用CT检测,47例采用MRI检测,35例患者接受了CT与MRI检测,参照病理检测结果,分析CT与MRI的检测准确性。结果 CT定位检测准确率为94.3%,对良性胆道梗阻定性检测准确率为78.9%,对恶性胆道梗阻定性检测准确率为73.5%;MRI对良性胆道梗阻定性检测准确率为81.3%,对恶性胆道梗阻定性检测准确率为87.1%;CT联合MRI定位检测准确率为100.0%,对良性胆道梗阻定性检测准确率为92.3%,对恶性胆道梗阻定性检测准确率为定性检测准备率为95.5%。结论 CT与MRI对良恶性胆道梗阻诊断方面各有长处,联合使用CT与MRI对患者进行检测并对比分析影像学结果能有效提高CT、MRI对良恶性胆道梗阻的定位与定性检测准确性。

Objective To study the value of CT and MRI in diagnosis of benign and malignant biliary obstruction, to provide theoretical basis for accuracy for improve the clinical diagnosis of benign and malignant biliary obstruc- tion. Methods Retrospective analysis the clinical data of 135 patients with biliary obstruction,who got treatment in May 2012 - December 2013 in our hospital. Which 48 cases is benign and 87 cases is malignant, 53 cases were treated by CT detection, 47 patients with MRI examination, 35 patients underwent CT and MRI detection, refer- ence to pathology results, analysis the testing accuracy of CT and MRI. Results CT positioning accuracy was 94.3% , the accuracy rate for benign biliary obstruction qualitative detection was 78.9% , the accuracy rate for qualitative detection of malignant biliary was 73.5% ;The accuracy rate of MRI for benign biliary obstruction quali- tative was 81.3% , the qualitative detection of malignant biliary obstruction rate was 87.1% ; Detection accuracy was 100.0% , the positioning of the CT in combination with MRI benign biliary obstruction qualitative detection ac- curacy rate was 92.3 %, the qualitative detection accuracy for the qualitative detection of malignant biliary obstruc- tion rate was 95.5%. Conclusion CT and MRI have each strengths for benign and malignant biliary obstruction diagnosed, combination of CT and MRI to detected for patients and analysis of imaging results can effectively im- prove the CT and MRI for the positioning and qualitative.
