COPD急性加重期患者PCT水平的影响因素及相关性分析

The influence factors of PCT level in COPD acute exacerbations patients and its correlation analysis

目的研究慢性阻塞性肺疾病(COPD)急性加重期患者降钙素原(PCT)水平的影响因素及相关性。方法将收治的60例COPD急性加重期患者纳入研究对象。根据PCT水平不同分为观察组30例和对照组30例,观察组患者血清PCT≥1.5 ng/m L,对照组患者血清PCT<1.5ng/m L,检测2组患者的肺功能指标以及血清炎症因子含量。结果观察组患者FEV1.0/FVC、PEF、MMRC、6 MWD均低于对照组(P<0.05);观察组患者血清IL-1β、IL-8、TNF-α、hs-CRP水平均高于对照组(P<0.05);FEV1.0/FVC、PEF、MMRC、6 MWD与PCT水平呈正相关(P<0.05),血清IL-1β、IL-8、TNF-α、hs-CRP水平与PCT呈负相关(P<0.05)。结论 COPD急性加重期患者PCT水平受到肺功能和血清炎症因子含量的影响;针对肺功能和血清炎症因子含量低的COPD患者,PCT检查结果阴性时仍应注意病情发展。

Objective To study the influence factors of PCT level of patients with COPD acute exacerbations and their correla- tion. Methods 60 cases patients with COPD acute exacerbations were enrolled and divided into two groups according to calcitonin level- sPCT ≥ 1.5 ng/ml in observation group and PCT 〈 1.5 ng/ml in control group. The indices of pulmonary function and the contents of inflammatory factors in serum were detected. Results FEV1/FVC,PEF,MRC,6 MWD in observation group were lower than those in control group （ P 〈 0.05 ）. The levels of IL - 113, IL - 8, TNF - or, hs - CRP in serum were higher than those in control group （ P 〈 0.05 ）. The levels of PCT in serum negatively correlated with FEV1/FVC, PEF （ P 〈 0.05 ）, MMRC ,6 MWD and positively correlated with the levels of IL - 113, IL - 8, TNF -α, hs - CRP in serum （ P 〈 0.05 ）. Conclusion PCT level in COPD acute exacerbations patients is affected by pulmonary function and the inflammatory cytokines in serum, for good lung function, low serum inflammatory factors content COPD patients, negative PCT results should be paid more attention to the development of the disease.
