腹腔镜联合胆道镜治疗胆囊胆总管结石与开腹手术疗效比较的回顾性研究

目的探讨腹腔镜联合胆道镜治疗胆囊合并胆总管结石的临床疗效比较。方法随机选取80例胆囊结石并胆总管结石患者并随机分腔镜组(40例)与对照组(40例),腔镜组行腹腔镜联合胆道镜治疗,对照组行开腹手术,然后对比两组疗效。结果两组均全部取石成功,且术后无严重并发症;腔镜组手术时间明显长于对照组(t=14.51,P<0.05),术中出血量(t=51.74,P<0.05),肛门排气时间(t=8.63,P<0.05),总住院时间均明显低于对照组(t=14.51,P<0.05);:对照组IL-2术后较术前下降(P<0.05),IL-2术后较腔镜组术后降低(P<0.05)。IL-6术后较术前明显上升(P<0.05)。IL-6术后对照组较腔镜组上升(P<0.05)。腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石疗效好,患者住院时间,出血量,并发症少。

Objective To compare the clinical effect of laparoscopie cholecystectomy and common bile duct stones in the treatment of gallbladder and common bile duct stones. Methods We randomly selected 80 cases of cholecystolithiasis and choledocholithiasis patients and were randomly divided into laparoscopic group （40 cases） and control group （40 cases） , laparoscopic group underwent laparoscopy combined with choledochoscopy in treatment, while the control group underwent laparotomy, and then compare the two groups of curative effect. Results Two groups were all successful, and after the operation without serious complications; laparoscopic operation time was significantly longer than that of the control group （ t = 14. 51, P 〈 0.05 ）, intraoperative blood loss （ t = 51.74, P 〈0. 05）, anal exhaust time （8.63, P 〈0. 05）, total length of hospital stay were significantly lower than that of the control group （t = 14. 51, P 〈0.05） ; compared with the preoperative fell after operation in control group IL-2 （P 〈0. 05） and IL-2 after operation than laparoscopic group were lower （P 〈 0. 05 ）. IL-6 was significantly higher than that before operation （P 〈 0.05）. After IL- 6 group compared with the laparoscopic group increased （P 〈 0.05 ）. Conclusion Laparoscopy combined with biliary endoscopy in the treatment of gallbladder stone with common bile duct stones, good curative effect, the time of hospitalization, the amount of bleeding, less complications.
