螺旋CT三维重建技术在诊断腰椎滑脱症中的应用价值 被引量:2

摘要 目的探讨螺旋CT三维重建技术在腰椎滑脱症诊断中的临床应用效果。方法选择本院放射科及广州市白云区人民医院放射科的56例腰椎滑脱症病人的x线片、CT及螺旋CT三维重建图像,结合术中椎弓根钉定位及对于是否为峡部裂的不同处理,分析螺旋CT三维重建在腰椎滑脱症中的诊断价值。结果56例中2例为侧方滑脱,其他均为腰椎前滑脱,三种检查方法对椎弓峡部的成像存在不同,螺旋CT三维重建可清楚显示椎弓峡部的全貌和裂隙,指导手术定位。结论螺旋CT三维重建能较好地评价椎弓峡部裂所致的病理解剖改变,科学合理地完善手术诊疗方案,通过CT图像三维重建技术充分显示病变,判断腰椎滑脱的性质与精确测量滑脱的程度,利于临床制定合理的诊疗方案,具有很大的优越性及应用前景。 Objective To investigate the application value of spiral CT three-dimensional reconstruction technique in the diagnosis of lumbar olisthe disease. Methods X-ray film, CT image, and spiral CT three- dimensional reconstruction image of 56 cases of lumbar olisthe disease from department of radiology of our hospital and Baiyun District People's Hospital were selected, combined with intraoperative pedicle screw position and different processing of spondylolysis, analyzed the value of spiral CT three-dimensional reconstruction in the diagnosis of lumbar olisthe disease. Results There were 2 cases of lateral slip and 54 cases of olisthesis in 56 patients. The imaging of vertebral arch isthmus by three methods of examination were different. Spiral CT three-dimensional reconstruction could clearly show the overall perspective and cracks of spondylolysis to guide operation positioning. Conclusions Spiral CT three-dimensional reconstruction can well evaluate pathological anatomy changes caused by spondylolysis, improve the surgical diagnosis and treatment plan scientifically and reasonably. Through lesions fully showed by CT image three-dimensional reconstruction technology, judging the nature of lumbar spondylolisthesis and accurately measuring the degree of slip, are conducive to the formulation of reasonable clinical diagnosis and treatment plan, with great advantages and application prospects.
