益气补脾凉血汤联合西药治疗IgA肾炎随机平行对照研究

[目的]观察益气补脾凉血汤联合西药治疗Ig A肾炎疗效。[方法]使用随机平行对照方法,将80例住院患者按病志号抽签方法简单随机分两组。对照组40例阿司匹林,100mg/次,1次/d;氯沙坦,50mg/次,1次/d,中、重度高血压可根据实际情况加药量至100mg/次,1次/d;扁桃体炎或咽炎,白细胞高,阿奇霉素,0.5g/次,连服3d。治疗组40例益气补脾凉血汤(荆芥15g,银花、连翘各9g,蝉蜕、僵蚕、玄参、黄芩各6g,赤芍9g,白茅根6g;水肿严重加茯苓皮,薏苡仁,冬瓜皮等;咽痛严重酌加蒲公英,射干,胖大海等;血尿明显加大、小蓟,旱莲草,茜草等),1剂/d,水煎300m L,早晚口服;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、血肌酐、尿红细胞、尿蛋白、不良反应。连续治疗2疗程,判定疗效。[结果]临床疗效治疗组优于对照组(P<0.05)。血肌酐治疗组明显改善(P<0.05),对照组无显著变化(P>0.05);尿红细胞、尿蛋白两组均改善(P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。[结论]益气补脾凉血汤联合西药治疗Ig A肾炎,疗效满意,无严重不良反应,值得推广。

Treatment group 40 cases of YQBPLXT （herba schizonepetae 15 g, honeysuckle, forsythia each 9 g, cicada each 6 g, batryticated silkworm, radix scrophulariae, radix scutellariae, radix paeoniae rubra 9 g, 6 g cogongrass rhizome; edema and tuckahoe peel seriously, coix seed, such as wax gourd peel; serious sore throat to make considered additions dandelions, rhizoma belamcandae, boat-fruited sterculia etc; hematuria significantly increased, field thistle, dry grass, madder, etc.）, l/d, Fried 300 mL water, morning and evening oral; Western medicine treatment with the control group. Continuous treatment of 7 d for 1 course of treatment. Observation of clinical symptoms, serum creatinine, urine red blood cells and urine protein, adverse reactions. Continuous treatment 2 period of treatment, judge curative effect.[Results] The clinical efficacy of the treatment group is better than that of control group （P〈0.05）. Serum creatinine in treatment group improved significantly （P〈0.05）, the control group had no significant change （P〉0.05）, urine red blood cells and urine protein in the two groups were improved （P〈0.01）, the treatment group is better than that of control group （P〈0.05, P〈0.01）. [Conclusion] YQBPLXT combined western medicine treatment of IgA glomerulonephritis, curative effect is satisfied, no serious adverse reactions, worthy of promotion.
