摘要
目的探讨鲑降钙素联合红骨髓局部经皮骨膜下注射或髓腔注射治疗骨折延迟愈合及骨不连的临床疗效。方法选取我院骨科2006年5月至2014年2月收治骨折后骨延迟愈合及骨不连患者53例,随机分成治疗组和对照组两组,治疗组26例,对照组27例。治疗组给予鲑降钙素和自体新鲜红骨髓经皮骨膜下或髓腔穿刺注射,对照组给予自体红骨髓穿刺注射。注射时严格无菌,首次注射均在C型臂透视下进行。10 d治疗一次,3次为一个疗程。治疗后进行为期半年的随访,比较两组患者骨骼愈合情况。结果随访6个月时,治疗组总有效率达到96.2%,对照组为44.4%;随访结束时,治疗组平均愈合时间(4.7±4.9)个月,对照组(8.2±6.3)个月。治疗组骨骼愈合速度及愈合效果明显好于对照组。结论鲑降钙素联合红骨髓局部经皮骨膜下注射或髓腔注射治疗骨折延迟愈合及骨不连操作简单、价格低廉且疗效显著,适用于各级医院尤其是基层医院。
Objective To investigate clinical effect of the salmon calcitonin combined with red bone marrow local via percutaneous subperiosteum injection or the marrow cavity injection in treatment of delayed healing and nonunion of fracture.Methods 53 cases of delayed healing of bone nonunion and bone nonunion were divided into treatment group and control group. Treatment group received salmon calcitonin and fresh autologous red bone marrow by subperiosteal skin or medullary cavity puncture. And autologous red bone marrow puncture injection was given to the patients in the control group. The first injection was performed under the Carm. There were 10 days between treatments and 3 times for a course of treatment. The patients were followed up for half a year,the bone healing of the two groups were compared. Results At 2 months follow-up,the total effective rate of the treatment group was 96. 2%,while the control group was 29. 6%,and the healing rate of the treatment group was higher than that of the control group. The total effective rate was 96. 2% in the treatment group and 44. 4% in the control group after 6 months of follow-up. The treatment group was significantly better than the control group. Conclusion The salmon calcitonin combined with red bone marrow local via percutaneous subperiosteum injection or the marrow cavity injection for che treatment of delayed fracture healing and bone nonunion operation is simple. The price is low and the effect is significant.
