静脉用药微粒污染对急性呼吸窘迫综合征患者微循环的影响 被引量:1

摘要 目的探讨静脉用药微粒污染对入住重症监护病房(ICU)的急性呼吸窘迫综合征(ARDS)患者微循环的影响。方法收集2013-2015年因合并ARDS入住本院ICU的患者共50例,按静脉用药分别采用直流过滤输注控制微颗粒以及普通静脉输注平均分为试验组及对照组,比较2组间相关因素的差异。结果单因素分析结果显示,2组入住ICU时间、持续发热时间、白细胞介素-8(IL-8)的差异有统计学意义;进一步通过非条件Logistic回归分析发现,采用直流过滤输注对缩短入住ICU时间[OR(95%CI)=0.212(0.094,0.468)]、降低IL-8[OR(95%CI)=0.315(0.25,0.715)]的关联有统计学意义(P<0.05)。结论对ARDS患者静脉用药时采用直流过滤输注控制微粒污染对微循环有积极的影响。 Objective To investigate the effect of particles contamination of intravenous infusion on microcirculation in ARDS patients in ICU.Methods A total of 50 cases of patients with ARDS admitted in ICU between 2013 and 2015were collected prospectively.According to intravenous medication,patients were divided into the experimental group(in-line filtration perfusion was used to control microparticles)and the control group(common perfusion were included),25 cases in each.The relevant factors were compared between the two groups.Results Univariate analysis showed that the differences in length of ICU stay,duration of fever and IL-8between the two groups were significant.Further unconditional logistic regression analysis showed that the correlation of in-line filtration perfusion with decrease of length of ICU stay [OR(95%CI)=0.212(0.094,0.468)]and decrease of IL-8[OR(95%CI)=0.315(0.25,0.715)]were significant(P〈0.05).Conclusion The application of in-line filtration perfusion in patients with ARDS to control particles contamination has positive effect on pulmonary microcirculation.
