降钙素原联合内毒素鲎试验在儿童社区获得性肺炎中的临床应用

The clinical application of procalcitonin and endotoxin limulus in community- acquired pneumonia of children

目的:探讨降钙素原(PCT)联合内毒素鲎试验在儿童社区获得性肺炎临床诊断中的应用价值。方法:选取2014年1月—2015年12月接诊的120例社区获得性肺炎患儿,根据病原学检查结果将其分成细菌组与非细菌组,将细菌性组患儿再分为革兰阴性菌感染组(A组)和非革兰阴性菌感染组(B组),将所有患儿进行降钙素原、内毒素鲎定量测定检验,观察检验结果。结果:细菌组降钙素原阳性率明显高于非细菌组,差异有统计学意义(P<0.05)。通过内毒素鲎试验发现A组患儿的血清内毒素变化明显,B组患儿血清内毒素无明显变化,两组患儿对比差异有统计学意义(P<0.05)。结论:血清降钙素原对儿童社区获得性肺炎的早期诊断有较大的临床实用价值,内毒素鲎定量测定可以确定细菌的种类,防止抗生素类药物的错用,减少因用药的方向错误而影响治疗效果,对于临床用药有重要的参考价值。

Objective： To evaluate the clinical diagnostic value of procalcitonin（ PCT） combined with endotoxin Limulus test in children community- acquired pneumonia. Methods： From January 2014 to December 2015,admissions 120 cases of community- acquired pneumonia in children,according to etiology findings will be divided into groups of bacteria and non-bacterial group,the group of children with bacterial subdivided into Gram- negative bacteria infection group（ A group） and non- Gram- negative bacteria infection group（ group B）,all children with procalcitonin,quantitative determination of endotoxin limulus test,observe the test results. Results： According to test results can be seen in children with positive rates of bacterial infection procalcitonin group was higher than that of children of non- bacterial group,the comparison of the data between the two groups of children was significantly（ P 〈 0. 05）,with statistical significance. Endotoxin by the Limulus test,found that children with serum toxin A group changed significantly in the serum group B toxin no significant change,data comparison between the two groups of children was significantly（ P 〈0. 05）,with statistical significance. Conclusion： Serum procalcitonin for early diagnosis of children with community- acquired pneumonia have greater clinical value,quantitative LAL endotoxin assay can determine the type of bacteria,to prevent the wrong use of antibiotics,due to the direction of reducing medication errors effective treatment for clinical medicine has important reference value.
