摘要 目的探讨发光二极管(LED)蓝光治疗新生儿黄疸的疗效及安全性。方法 162例新生儿黄疸患儿随机分为LED组(n=80)和荧光管(CFT)组(n=82)。LED组采用LED蓝光治疗,CFT组采用CFT蓝光治疗。根据胎龄LED组又分为早产儿LED组32例和足月儿LED组48例,CFT组分为早产儿CFT组31例和足月儿CFT组51例。监测各组患儿光疗后4、8 h体温、经皮测胆红素值,观察光疗后8 h皮疹情况、尿量及大便次数。结果光疗后4、8 h,2组早产儿之间及2组足月儿之间的体温、经皮胆红素值差异无统计学意义(P>0.05)。光疗后8 h,2组早产儿之间及2组足月儿之间的大便次数、皮疹发生率比较,差异无统计学意义(P>0.05)。光疗8 h,早产儿LED组尿量显著高于早产儿CFT组(P<0.05);2组足月儿之间尿量比较,差异无统计学意义(P>0.05)。结论 LED与CFT蓝光治疗新生儿黄疸均具有一定疗效,LED蓝光治疗能够更有效控制患儿的体液代谢。 Objective To investigate the efficacy and safety of blue light-emitting diodes( LED) as light source in phototherapy for neonatal jaundice. Methods A total of 162 neonates with neonatal jaundice were randomly divided into LED group( n = 80) and fluorescent tube( CFT) group( n = 82),with blue light-emitting diodes and fluorescent tube as light source respectively. According to the gestational age,neonates in LED group were assigned to premature LED group( n = 32) and full-term LED group( n = 48),and neonates in CFT group were assigned to premature CFT group( n= 31) and full-term CFT group( n = 51). The primary outcomes were body temperature and transcutaneous bilirubin at 4 and 8 hours after phototherapy and incidence of rash,urine volume and defecation frequency 8 hours after phototherapy. Results There was no significant difference in body temperature and transcutaneous bilirubin at 4 and 8 hours after phototherapy between premature LED group and premature CFT group,so was not between full-term LED group and full-term CFT group( P >0. 05). There was no significant difference in incidence of rash and defecation frequency 8 hours after phototherapy between premature LED group and premature CFT group,so was not between full-term LED group and full-term CFT group( P > 0. 05). The urine volume was significant higher in premature LED group than that in premature CFT group( P < 0. 05),but with no significant difference between full-term LED group and full-term CFT group( P > 0. 05). Conclusion Both blue LED and CFT as light source shows efficacy and safety in phototherapy for neonatal jaundice,LED is more effective in regulating body fluid metabolism.
