无痛分娩前心理护理对初产妇产程及母婴结局的影响

Influence of psychological intervention before painless childbirth on duration of labor and maternal-infant outcome of primipara

目的探讨无痛分娩前心理护理对初产妇产程及母婴结局的影响。方法 100例行无痛分娩的初产妇随机分为对照组和观察组,各50例。对照组产妇分娩前给予常规护理,观察组在对照组基础上给予心理护理。比较2组产妇的产程、产后出血量、并发症发生情况及新生儿相关情况。结果观察组产妇的第一产程、第二产程、总产程时间及出血量显著低于对照组(P<0.01),观察组产后并发症发生率8.00%、新生儿窒息率4.00%、新生儿黄疸率2.00%,分别低于对照组的24.00%、18.00%和14.00%,差异有统计学意义(P<0.05)。结论初产妇无痛分娩前给予心理护理干预,能有效缩短产妇产程,改善母婴结局。

Objective To investigate the influence of psychological intervention before pain-less childbirth on duration of labor and maternal-infant outcome of primipara. Methods A total of 100 primiparas were randomly divided into the control group and the observation group, with 50 cases in each group. Both two groups were given conventional treatment before painless childbirth, and the observation group was given psychological intervention additionally. The duration of labor, postpartum hemorrhage, complication and neonatal status were compared between two groups. Results The observation group had a shorter duration of labor at first stage, second stage and overall duration , as well as less amount of hemorrhage, compared with the control group（P 〈0.01）. The incidence of complication ,neonatal asphyxia and neonatal jaundice was 8. 0 0 % , 4. 0 0 % and 2. 0 0 % in the observation group, and was 24.00% , 18.00% and 14. 0 0 % in the control group, with a significant difference（P 〈0. 05 ） . Conclusion Psychological intervention before painless childbirth can effectively shorten the duration of labor and improve the maternal-infant outcome of primipara.
