社区老年慢性病健康管理模式研究

Research on Health Management Mode of Community Elderly Chronic Disease

目前老年慢性病的发生率逐年升高,对其预防和治疗也经历了一个发展的过程,社区作为老年人活动的基本场所,在慢性病防治中发挥着基础性的作用,该文通过当前我国社区老年慢性病健康管理中存在的一些问题进行了分析,针对健康管理的特点,提出相应的解决方案,以研究符合我国的社区老年慢性病健康管理模式。

At present, the incidence rate of elderly chronic disease gradually increases, and the prevention and treatment of it has experienced a development process. As a basic place for the activities of the elderly, community plays a basic role in the prevention and cure of chronic disease. The paper analyzes some problems existing in the health management of com-munity elderly chronic disease of our country at present and provides corresponding solutions aiming at the health manage-ment characteristics in order to research the health management mode meeting the community elderly chronic disease of our country.
