无保护会阴接生法辅以会阴按摩及心理疏导的临床应用

Clinical application of unprotected perineum midwives with perineum massage and psychological counseling

目的：探讨无保护会阴接生法辅以会阴按摩、心理疏导在临床中的应用效果。方法：将2013年6月~2014年6月174例在我院行孕期检查及分娩无并发症的孕妇随机等分为观察组和对照组，观察组采用无保护会阴接生法辅以会阴按摩、心理疏导；对照组则按传统常规方法接生。观察两组产妇分娩和会阴情况以及新生儿结局。结果：观察组孕妇第二产程用时、产时出血量和产后2 h 出血量均明显少于对照组；产时疼痛程度轻于对照组；观察组产妇会阴侧切、分娩恐惧及新生儿产瘤发生率低于对照组，两组比较差异均有统计学意义（P ＜0．05）。结论：无保护会阴接生法辅以会阴按摩、心理疏导能有效缓解孕妇分娩的恐惧感和产时疼痛程度，减少产后出血量，降低会阴损伤程度，并缩短第二产程，同时也能影响新生儿结局。

Objective：To explore the application effects of unprotected perineum midwives with perineum massage and psychological counseling in clinical treatment. Methods：Selected 174 pregnant women conducting pregnancy examination and delivery without complication in our hospital during June 2013 -June 2014,randomly divided them into observation group and control group. Adopted unprotected perineum midwives with perineum massage and psycho-logical counseling to the observation group;delivered puerperae in the control group with traditional routine method. Observed puerperae delivery and peri-neum condition as well as newborn outcome in the two groups. Results：Time of puerperae’s second stage of labor,delivery bleeding amount and postopera-tive 2 - hours bleeding amount were all significantly less than those of the control group;the delivery pain degree was lighter than that of the control group;episiotomies,fear of delivery and newborn caput succedaneum of the observation group were all lower than those of the control group,the differences of the two groups were statistical significance（P 〈 0. 05）. Conclusion：Unprotected perineum midwives with perineum massage and psychological counseling can effectively relieve pregnant women’s delivery fear and delivery pain degree,reduce postoperative bleeding amount,reduce perineum damage degree,short-en the second stage of labor and influence newbornsˊ outcome in the mean time.
