2015年浙江省金华市健康人群水痘-带状疱疹病毒抗体水平调查

Antibody Levels against Varicella-zoster Virus among a Healthy Population in Jinhua City of Zhejiang Province,2015

目的了解金华市健康人群水痘-带状疱疹病毒（VZV）抗体水平。方法采集健康人群血清标本1 170份,采用酶联免疫吸附试验（ELISA）检测水痘IgG抗体。结果人群血清中VZV抗体阳性率为62.99%,几何平均浓度（GMC）为202.59 m IU/ml。各年龄组VZV抗体阳性率及GMC总体上呈随年龄增长而上升的趋势,8月龄~1岁组人群的抗体阳性率（13.33%）和GMC（28.53 m IU/ml）最低。在8月龄~14岁人群中,接种0、1和2剂次水痘疫苗（VarV）人群的VZV抗体阳性率分别为28.79%、40.66%、48.16%（χ~2=15.652,P〈0.001）;GMC分别为55.52m IU/ml、78.18 m IU/ml、175.38 m IU/ml（F=4.677,P=0.010）。2015年水痘年龄别发病率与抗体阳性率呈负相关（r=-0.745,P=0.003）。结论 VZV在人群中的感染率较高,接种1剂次VarV产生的保护效力不足以控制水痘疫情,建议采取2剂次VarV免疫策略。

Objective To determine antibody levels of varicella-zoster virus（ VZV） among a healthy population in Jinhua city.Methods 1 170 serum samples were collected from healthy people,and their VZV Ig G antibody levels were tested using enzyme-linked immunosorbent assay（ ELISA）.Results The positive rate and the geometric mean concentration（ GMC） of VZV antibody among all samples were62.99% and 202.59 m IU/ml,respectively.Age-specific positive rates and GMCs showed an increasing trend with age,with the lowest positive rate（ 13.33%） or GMC（ 28.53 m IU/ml） level being among 8-month to 1-year olds.Among children 8 months to 14 years of age who received 0,1,or 2 doses of varicella vaccine（ VarV）,the respective positive rates of VZV antibody were 28.79%,40.66% and48.16%（ χ~2= 15.652,P 0.001）,and the respective GMCs were 55.52 m IU/ml,78.18 m IU/ml and 175.38 m IU/ml（ F = 4.677,P = 0.010）.Age-specific incidence rates of varicella and age-specific VZV antibody positive rates had a negative correlation（ r =-0.745,P = 0.003）.Conclusions The VZV infection rate was high in this healthy population.The efficacy of 1 dose of VarV is not sufficient to control varicella.We recommend a 2-dose VarV immunization policy.
