瘢痕子宫再次妊娠产妇经阴道分娩的安全性

The security of vaginal delivery in pregnant women with repregnancy of cicatrix uterus

目的探究分析瘢痕子宫再次妊娠经阴道分娩的安全性。方法选取2015年1~10月在我院收治的瘢痕子宫再次妊娠产妇202例作为研究对象,随机分为研究组102例行阴道分娩,对照组100例行剖宫产。观察两组产妇的妊娠情况,妊娠结局（胎儿窘迫、宫缩乏力、子宫破裂、尿潴留、产后出血）及并发症等情况,分析瘢痕子宫再次妊娠经阴道分娩孕妇试产存在的风险及防范措施,并通过统计学比较两组的分娩情况以及并发症发生率。结果研究组与对照组产妇的产程时间、产时出血量、新生儿Apgar评分、新生儿体质量及住院时间比较,差异均具有统计学意义（P〈0.05）;研究组与对照组产妇产后的细菌感染、子宫破裂、产后出血、尿潴留、新生儿窒息等并发症比较,差异均具有统计学意义（P〈0.05）。结论瘢痕子宫再次妊娠产妇经阴道分娩可以显著减少分娩时出血量、缩短住院时间,安全可行,值得在临床上大力推广应用。

Objective To explore and analyze the security of vaginal delivery in repregnancy of cicatrix uterus. Methods 202 pregnant women with repregnancy of cicatrix uterus treated in our hospital from January 2015 to October 2015 were selected as the research objects, who were randomly divided into 102 cases of the study group with vaginal delivery and 100 cases of the control group with cesarean section. In the two groups, the pregnancy, the pregnancy outcome of fetal distress, uterine inertia, uterine rupture, urinary retention, and postpartum hemorrhage and complications were observed, the risk in vaginal delivery of pregnant women trial of labor with repregnancy of cicatrix uterus and preventive measures were analyzed, the delivery conditions and the incidence rate of complications were statistically compared. Results Between the study group and control group, the labor duration, quantity of intrapartum hemorrhage, neonatal Apgar score, neonatal birth weight and length of stay were compared, the differences were all statistically significant（P 0.05）, the complications of postpartum bacterial infection, uterine rupture, postpartum hemorrhage, urinary retention, and neonatal asphyxia were compared, the difference was statistically significant（P 0.05）. Conclusion The vaginal delivery of pregnant women with repregnancy of cicatrix uterus could significantly reduce the amount of bleeding during childbirth, shorter length of stay, have safety and feasibility, which was worthy of clinical vigorously promotion and application.
