肺表面活性物质联合鼻塞持续正压通气治疗新生儿呼吸窘迫综合征临床分析 被引量:10

摘要 目的观察分析新生儿呼吸窘迫综合征应用肺表面活性物质联合鼻塞持续正压通气治疗的临床效果。方法回顾性分析我院自2013年1月~2015年1月收治的100例新生儿呼吸窘迫综合征病例,根据治疗方法不同分为观察组(n=50)及对照组(n=50),对照组患儿给予鼻塞持续正压通气治疗,观察组患儿采用肺表面活性物质联合鼻塞持续正压通气治疗,科学评估两种患儿临床治疗效果。结果治疗6、24h后,两组患儿血气指标明显改善,观察组患儿PaCO_2低于对照组,pH值、PaO_2、PaO_2/FiO_2较对照组明显提高(P〈0.05);观察组患儿治疗总有效率为96.00%,高于对照组80.00%(P〈0.05);观察组患儿并发症发生率、氧疗时间低于对照组(P〈0.05)。结论在新生儿呼吸窘迫综合征治疗中,肺表面活性物质联合鼻塞持续正压通气可明显减少呼吸窘迫症状,改善血气指标,降低并发症发生率,临床疗效安全显著,值得推广应用。 Objective To observe and analyze the clinical effect of pulmonary surfactant combined with nasal continuous positive airway pressure in treatment of respiratory distress syndrome of newborn. Methods 100 cases of respiratory distress syndrome of newborn cured in our hospital from January 2013 to January 2015 were retrospectively analyzed and divided into observation group and control group with 50 cases in each according to the different treatment methods. Children in control group were treated with nasal continuous positive airway pressure, and children in observation group were treated with pulmonary surfactant combined with nasal continuous positive airway pressure. The clinical effects of the children in the two groups were scientifically evaluated. Results After 6h, 24 h treatment, blood gas indexes of children in the two groups were significantly improved. The value of PaCO_2 of children of observation group was lower than that of control group. The values of pH, PaO_2, PaO_2/FiO_2 were significantly increased than those of control group(P 0.05). Total effective rate of observation group was 96.00%, which was higher than that of control group with 80.00%(P 0.05). Complication rate, oxygen therapy time of observation group were lower than those of control group(P 0.05). Conclusion In the treatment of respiratory distress syndrome of newborn, pulmonary surfactant combined with nasal continuous positive airway pressure can significantly reduce the symptoms of respiratory distress, improve blood gas indexes, and reduce the incidence of complications. Clinical efficacy is significant. It is worth popularizing and applying.
