以给药时间为中心的先天性心脏病CT扫描方案

摘要 目的:从给药时间入手,寻求一种兼顾图像质量和可操作性的先天性心脏病CTA扫描方案。方法:55例先天性心脏病患者分为动态监测法组(19例)、目测手动触发法组(18例)和给药时间为中心法组(18例)扫描,对比三种方法射线束硬化伪影的评分和心室腔对比剂浓度。结果:给药时间为中心法组射线束硬化伪影评分优于动态监测法组和目测手动触发法组,右心室腔浓度给药时间为给药时间为中心法组低于动态监测法组和目测手动触发法组,左心室腔浓度给药时间为中心法组高于目测手动触发法组与动态监测法组,但差异均无统计学意义(P>0.05)。结论:给药时间为中心法综合考虑了机器的性能、给药的速度、扫描所需的时间等参数,将扫描触发时间采用公式量化,使先天性心脏病的CT扫描更加容易掌握,并且其对射线束硬化伪影的抑制明显优于传统方案。 Objective: To seek a congenital heart disease ( CHD ) CTA scanning plan combining with image quality and operability on the basis of administrating time.Method: Scans were performed on 55 CHD patients through three groups, including dynamic monitoring method group of 19 patients, visual with manual triggering method group of 18 patients, and administration-time-centered method group of 18 patients, then beam hardening artifact score and ventricular eavity's contrast media concentration among the above mentioned three ways were evaluated.Result: The beam hardening artifact score in the administration-time-centered method was better than that of both dynamic monitoring group and visual with manual triggering group.With respect to contrast media concentration of right ventricular cavity, the figures in the administration-time-centered group was lower than that in the dynamic monitoring group and visual & manual triggering group.While the figures in the concentration of left ventricular cavity in the administration-time-centered group were higher than that in the visual with manual triggering group but showed no statistical difference compared with the figures in the dynamic monitoring group. Conclusion : Administration-time-centered method integrates parameters such as performance of machine, speed of drug administration, and scanning time into account, adopts formula to quantize scan triggering time, and makes CHD CT scanning more easily to be grasped.In addition, its restriction on beam hardening artifact is superior to traditional plan, therefore this method can be further studied and applied in clinical field.
