摘要 目的探讨半乳糖凝集素-3(Gal-3)在急性心肌梗死(AMI)所致的不同级别心力衰竭中的表达及其临床意义。方法将180例AMI患者按在院期间是否并发心力衰竭分为心力衰竭组87例与非心力衰竭组93例,其中87例心力衰竭患者NYHA分级Ⅰ级26例,Ⅱ级38例,Ⅲ级-Ⅳ级23例,测定患者血清Gal-3水平。结果治疗前心力衰竭组血清Gal-3水平明显高于非心力衰竭组(P<0.05)。治疗后心力衰竭组血清Gal-3水平均明显低于治疗前(P<0.05);治疗后心力衰竭组血清Gal-3水平明显高于非心力衰竭组(P<0.05)。随着NYHA分级的增加,血清Gal-3水平逐渐升高(P<0.05);将心力衰竭组血清Gal-3水平及NYHA分级进行相关性分析,结果显示两者呈正相关(r=0.362,P=0.002)。结论 Gal-3与AMI患者的心功能有着密切的关系,能够较好地预测急性心肌梗死患者发生心力衰竭的可能性以及严重程度,对判断预后有着重要的临床价值。 Objective To investigate the expression and clinical significance of-3 (Gal-3) in different levels of heart failure caused by acute myocardial infarction(AMI). Methods 180 patients with AMI were divided into heart failure group(87 cases) and non heart failure group(93 cases). Of these 87 patients with heart failure,26 patients were grade NYHA,38 grade II and 23 grade III grade II. Serum Gal-3 levels were determined. Results The serum Gal-3 level in heart failure group was significantly higher than that in non heart failure group (P〈0.05). The serum Gal 3 level was significantly lower in the treatment group than in the treatment group (P〈0.05). The serum Gal-3 level was signifi-cantly higher in the treatment group than in the non heart failure group (P〈0.05). With the increase of NYHA grade, serum Gal-3 level gradually increased (P〈0.05). The correlation between serum Gal-3 level and NYHA level in heart failure group was analyzed. The results showed that there was a positive correlation(r= 0. 362,P=0. 002). Conclusion Gal-3 has a close relationship with heart function in patients with AMI,and it can predict the probability of heart failure and the severity of heart failure in patients with acute myocardial infarction.
