Ⅱ型肱骨髁上骨折中医手法整复夹板外固定与闭合穿针内固定的疗效对比分析

The curative effect comparative analysis of TCM manipulation reduction of splint external fixation and closed pinning internal fixation on type-Ⅱ supracondylar humeral fractures

目的对比研究中医手法整复夹板外固定与闭合穿针内固定对Ⅱ型肱骨髁上骨折的临床疗效。方法选取2013年10月-2015年6月80例Ⅱ型肱骨髁上骨折患者进行分组对比研究。采取数字抽签法分为中医组和手术组,各有40例。中医组进行中医手法整复夹板外固定治疗,手术组进行闭合穿针内固定治疗。对比两组患者的治疗效果。结果解除固定时两组患者的B角、C角存在明显差异,差异具有统计学意义（P〈0.05）;与复位后摄片时相比,解除固定时两组患者的B角出现明显的变化,,差异具有统计学意义（P〈0.05）。手术组患者的肘内翻发生率与中医组相比更低,差异无统计学意义（P〉0.05）。结论中医手法整复夹板外固定与闭合穿针内固定治疗Ⅱ型肱骨髁上骨折均具有显著的临床疗效,但闭合穿针内固定治疗在维持骨折端复位及减少肘内翻方面更有优势。

Objective To compare and study the clinical curative effect of TCM manipulation reduction of splint external fixation and closed pinning internal fixation on type-Ⅱ supracondylar humeral fractures. Methods 80 patients with type-Ⅱ supracondylar humeral fractures who were selected for group comparison study from October 2013 to June 2015 were divided into TCM group and operation group, 40 cases in each group, according to the digital draw method. The TCM group received treatment with TCM manipulation reduction of splint external fixation, while the operation group received therapy with closed pinning internal fixation. The treatment effects of the two groups were compared. Results To remove the fixed time, the B angle and C angle of the two groups had obvious difference, the difference was statistically significant（P〈0.05）, compared with the reset after the photo film, the B angle of the two groups had obvious change, the difference was statistically significant（P〈0.05）. The incidence rate of cubitus varus of the operation group was lower than that of the TCM group, but, the difference was not statistically significant（P〉0.05）. Conclusion TCM manipulative reduction of splint external fixation and closed pinning internal fixation on treating type-Ⅱ supracondylar humeral fractures both had significantly clinical curative effect, but the treatment of closed pinning internal fixation in maintaining fracture end reset and reducing cubitus varus had more advantages.
