复合式小梁切除术治疗慢性闭角型青光眼的眼压观察

Intraocular pressure observation of excision of compound trabeculectomy in treatment of chronic angle closure glaucoma

目的探讨复合式小梁切除术治疗原发性慢性闭角性青光眼效果。方法选取我院2012年1月-2015年12月收治的200例原发性慢性闭角性青光眼患者作为研究对象,随机分为观察组和对照组,每组各100例,观察组采取复合式小梁切除术,对照组则实行传统小梁切除术,比较两组患者手术的成功率、术后并发症发生率和术后患者的眼压情况。结果观察组患者术后的完全控制率明显高于对照组,差异具有统计学意义（χ^2=7.066,P〈0.05）;观察组的正常前房率为96.0%,对照组的正常前房率为76.0%,两组比较差异具有统计学意义（P〈0.05）;经过治疗,两组患者的平均眼压和治疗前相比均明显下降,差异具有统计学意义（P〈0.05）,且治疗后观察组的平均眼压均明显少于对照组,差异均具有统计学意义（P〈0.01）。结论对于原发性慢性闭角性青光眼术后初期浅前房、睫状环阻滞性青光眼的发生率以及术后滤过道瘢痕的发生来说,复合式小梁切除术可以起到显著降低的效果,因此有助于该疾病手术成功率的提高。

Objective To explore the effect of excision of compound trabeculectomy in treatment of primary chronic angle closure glaucoma. Methods 200 cases of patients with primary chronic angle closure glaucoma were selected as the research objects,which were cured in our hospital from January 2012 to December 2015.They were divided into observation group and control group with 100 cases in each.Patients in observation group were treated with excision of compound trabeculectomy.Patients in control group were treated with traditional trabeculectomy.Success rate of surgery,postoperative complication rate and intraocular pressure of patients after surgery of the two groups were compared. Results Postoperative complete control rate of observation group was significantly higher than that of control group.The difference was statistically significant（χ^2=7.066,P〈0.05）.Normal anterior chamber rate of observation group was 96.0%.Normal anterior chamber rate of control group was 76.0%.There was a great difference between the two groups（P〈0.05）.After treatment,compared with before treatment,mean intraocular pressure of the two groups were all significantly decreased.The differences were statistically significant（P 〈0.01）. Conclusion For the incidence rate of shallow anterior chamber,ciliary block glaucoma and occurrence of postoperative scar filtration canal at early primary chronic angle-closure glaucoma surgery,excision of compound trabeculectomy could play a significant reduction effect.So it could help to improve the success rate of the disease.
