香菇多糖联合顺铂治疗恶性胸腹水的效果

Effect of lentinan combined with Cisplatin in the treatment of malignant pleural effusion and ascites

目的探讨香菇多糖联合顺铂治疗恶性胸腹水的临床效果。方法选取2012年3月～2014年3月在唐山市人民医院接受治疗的124例恶性胸腹水患者，将其分为对照组和试验组，各62例。所有患者均接受引流治疗，在此基础上，对照组患者接受腔内注射顺铂治疗，试验组患者接受腔内联合注射香菇多糖和顺铂治疗。评估两组近期疗效及治疗后的生活质量。对所有患者进行为期2年的随访．记录患者2年内的存活率。记录放化疗过程中的不良反应发生情况。结果试验组的治疗效果显著优于对照组（u=6．79，P〈0．05），生存质量测定量表得分显著优于对照组（u=4．09，P〈0．05），存活率显著高于对照组（61．29％比22．58％，P〈0．05），患者出现恶心呕吐和腹泻的人数显著少于对照组患者（7例比18例，3例比9例，P〈0．05）。结论香菇多糖联合顺铂治疗恶性胸腹水效果显著，能增加患者的治疗顺应性，值得临床推广。

Objective To investigate the effect of lentinan combined with Cisplatin in the treatment of malignant pleu- ral effusion and ascites. Methods One hundred and twenty-four cases of patients with malignant pleural effusion and ascites treated in Tangshan People＇s Hospital from March 2012 to March 2014 were selected and divided into the con- trol group and the experimental group, with 62 cases in each group. All patients were treated with drainage. On this ba- sis, the control group received intrapleural injection of Cisplatin, the experimental group received combined injection of lentinan and Cisplatin. The short-term effect and the quality of life after treatment in the two groups were evaluated. All patients were followed up for 2 years, and the survival rate during 2 years was recorded. The occurrence of adverse reactions during radiotherapy and chemotherapy was recorded. Results The curative effect of the experimental group was significantly better than that of the control group （u = 6.79, P 〈 0.05）. The WHOQOL-BREF score of the experi- mental group was significantly better than that of the control group （u = 4.09, P 〈 0.05）. The survival rate of experi- mental group was significantly higher than that of the control group （61.29% vs 22.58%, P 〈 0.05）. The number of pa- tients with nausea and vomiting and diarrhea in the experimental group was significantly less than that in the control group （7 cases vs 18 cases, 3 cases vs 9 cases, P 〈 0.05）. Conclusion Lentinan combined with Cisplatin in the treat- ment of malignant pleural effusion and ascites is effective, which can increase the treatment compliance of patients. It is worthy of clinical promotion.
