延长头孢他啶输注时间治疗革兰阴性杆菌感染的临床效果 被引量:5

摘要 目的评价延长头孢他啶输注时间对革兰阴性杆菌感染患者的疗效及安全性。方法选择浙江省乐清市第二人民医院2013年9月~2015年3月需要给予头孢他啶连续治疗的患者213例。根据头孢他啶的输注时间分为三组,即30rain组、1h组和3h组。每组各71例。观察并比较三组患者的临床疗效、细菌学疗效以及不良反应发生率。结果完成疗程的患者183例,30min组57例,治疗有效率70.2%,细菌清除率64.7%,不良反应发生率3.51%;1h组62例,治疗有效率74.2%,细菌清除率66.7%,不良反应发生率3.23%;3h组64例,治疗有效率89.1%,细菌清除率80.O%,不良反应发生率3.16%。30min组与1h组临床有效率、细菌清除率以及不良反应发生率比较,差异无统计学意义(P〉0.05),30min组与3h组临床有效率比较,差异有统计学意义(P〈0.05),而细菌清除率与不良反应发生率差异无统计学意义(P〉0.05)。结论头孢他啶延长输注1h的方案与常规输注方案疗效相当,而延长输注3h的临床疗效优于常规输注。 Objective To evaluate the clinical efficacy and safety of extending the infusion time of ceftazidime in the treatment of Gram-negative bacteria infection. Methods Two hundred and thirteen patients with continuous treatment of eeftazidime in The second Yueqing People's Hospital of Zhejiang Province from September 2013 to March 2015 were divided to three groups: 30 min group, 1 h group and 3 h group, each group consists of 71 cases. The clinical efficacy, bacteriological efficacy and side-effects incidence rate of the three groups were observed and compared. Results Fifty- seven patients were enrolled in 30 min group with an effective rate of 70.2%, bacterial clearance rate of 64.7% and side-effects incidence rate of 3.51%. Sixty-two patients were enrolled in prolonged lh infusion group with an effective rate of 74.2%, bacterial clearance rate of 66.7% and side-effects incidence rate of 3.23%. Sixty-four patients were en- rolled in prolonged 3 h infusion group with an effective rate of 89.1%, bacterial clearance rate of 80.0% and side-ef- fects incidence rate of 3.16%. There were no significant differences between traditional 30 min infusion group and pro- longed 1 h infusion group on effective rate, bacterial clearance rate and side-effects incidence rate. There were signifi- cant differences between traditional 30 min infusion group and prolonged 3 h infusion group on effective rate. Conclu- sion Ceftazidime prolonged 1 h infusion group have equivalent efficacy to ceftazidime traditional 30 min infusion group, but the clinical efficacy of cefiazidime prolonged 3 h infusion group is superior to traditional 30 min infusion group.
