剖宫产产妇施行循证护理的价值评估及分析

Application value and analysis of evidence-based nursing in caesarean section parturient

目的 探讨循证护理在剖宫产产妇中的应用价值。方法 选取我院2014年9月-2015年9月收治的剖宫产产妇200例作为研究对象,将其随机分为两组,各100例,对照组在围术期给予常规护理,观察组在围术期采用循证护理模式。分别对两组产妇护理前后的心理状态、术后泌乳功能、康复情况（子宫复旧及排气、排便时间）、术后生存质量（不良反应发生情况与疼痛情况）及护理满意度等指标进行评估和比较。结果 观察组产妇在术中出血量（147.24±26.85）ml、手术时间（20.23±3.82）min、SHS发生率为3.00%、新生儿Apgar评分〉7分比例为69.00%、护理后的SAS评分为（45.26±2.41）分和SDS评分为（45.78±2.40）分、术后泌乳始动时间为（24.09±4.52）h、泌乳不足发生率为10.00%、术后24 h子宫复旧情况为（12.45±1.37）cm、排气时间为（23.52±2.18）h、排便时间为（3.23±0.44）h、术后不良反应发生率（低血糖、消化道不良反应、饥饿、切口感染发生率分别为1.00%、1.00%、2.00%、3.00%）、VAS疼痛评分为（4.0±1.1）分、疼痛难忍率为3.00%、护理满意度为98.00%等指标比较,均显著优于对照组,差异有统计学意义（P〈0.05）。结论 剖宫产产妇施行循证护理效果好,安全性佳,对母婴有重要意义。

Objective To explore the application value of evidence-based nursing in caesarean section parturient.Methods 200 caesarean section puerpera from September 2014 to September 2015 of our hospital were selected as study objects.Patients were randomly divided into two groups,and each group was 100 cases.Patients in control group were given routine nursing during perioperative period,and patients in observation group were given evidence-based nursing during perioperative period.The mentality,postoperative lactation function,rehabilitation situation （uterine involution and ex- haust,defecation time）, postoperative life quality （adverse reaction conditions and pain） and nursing satisfaction before and after operation in two groups were respectively evaluated and analyzed.Results The index of bleeding volume during operation,operation time,the incidence of SHS in supine position,the ratio of neonatal Apgar score〉7,SAS score after nursing,SDS score,the initial time of lactation after operation,incidence of lactation deficiency,Postoperative uterine involution in 24 h,exhaust time,defecation time,incidence rate of postoperative adverse reactions （hypoglycemia,digestive tract adverse reaction,hunger, incidence rate of incision infection）,VAS pain score,pain unbearable rate and nursing satisfaction in observation group were respectively （147.24±26.85）ml,（20.23±3.82）min,3.00%,69.00%,（45.26±2.41）,（45.78± 2.40）,（24.09±4.52）h,10.00%,（12.45±1.37）cm,（23.52±2.18）h,（3.23±0.44）h,1.00%, 1.00%, 2.00%, 3.00%,（4.0±1.1）,3.00% and 98.00%,which were obvious better than that of control group,and the difference was statistical significance （P〈0.05）. Conclusion Cesarean section maternal conducting evidence-based nursing has good effects and safety,and has great meaning for mothers and infants.
