摘要
目的探讨头孢哌酮舒巴坦联合果糖二磷酸钠对老年呼吸机相关肺炎伴低磷血症患者的临床疗效及安全性。方法选取温州医科大学附属第三医院治疗的呼吸机相关肺炎伴低磷血症老年患者68例,按随机数字表法进行分组,对照组34例予以常规治疗,研究组34例予以头孢哌酮舒巴坦联合果糖二磷酸钠治疗,治疗前后对比2组间血清离子水平、免疫水平及其他相应指标变化,同时对比临床疗效及不良反应。结果对照组治疗有效率67.65%低于研究组治疗有效率88.24%,差异具有统计学意义(P<0.05);与对照组比较,研究组治疗后血钠、磷、钾均升高,治疗后血清Lac、PCT、IL-6、CRP均降低,治疗后Ig A降低,Ig M、Ig G升高,差异具有统计学意义(P<0.05)。研究组不良反应率(8.82%)与对照组(5.88%)比较差异无统计学意义。结论采用头孢哌酮舒巴坦联合果糖二磷酸钠治疗老年呼吸机相关肺炎伴低磷血症疗效显著,安全性高。
Objective To investigate the efficacy and safety of cefoperazone/sulbactam combined with fructosediphosphate sodium on in elderly ventilator associated pneumonia with hypophosphatemia. Methods A total of 68 cases with elderly ventilator associated pneumonia with hypophosphatemia from our hospital were collected and randomly divided into experimental group and control group with 34 cases in each group. Patients in the control group were treated by conventional treatment; patients in the experimental group were treated by cefoperazone/sulbactam combined with fructose diphosphate sodium, the serum levels of hydronium, immune and other corresponding indexes were compared between the two groups before and after treatment, and the clinical efficacy and adverse reactions were compared. Results The effective rate of the control group 67. 65% was lower than the experimental group 88.24%, with statistical significance (P 〈 0. 05 ); compared with the control group, after treatment, the serum sodium, phosphorus and potassium of the experimental group increased, the serum Lac, PCT, IL-6 and CRP decreased, the IgA decreased, IgM and IgG increased, with statistical significance ( P 〈 0. 05 ). There was no significant difference in adverse reactions rate between the experimental group (8. 82% ) and control group .(5.88% ). Conclusion Cefoperazone/sulbactam combined with fructose diphosphate sodium has the obvious clinical efficacy in elderly ventilator associated pneumonia with hypophosphatemia, with high safety.
