两性霉素B联合自体血清注射治疗真菌性角膜溃疡的疗效

Clinical efficacy of amphotericin B combined with autologous serum injection in the treatment of fungal corneal ulcer

目的探讨两性霉素B联合自体血清注射治疗真菌性角膜溃疡的临床疗效及安全性。方法选取来浙江省杭州市余杭区第五人民医院眼科治疗并确诊的真菌性角膜溃疡患者82例单眼发病患者,按随机数字表法进行分组,对照组41例予两性霉素B治疗;研究组41例予两性霉素B联合自体血清注射治疗,记录2组患者前房积脓、角膜溃疡、异物感、畏光流泪等临床症状改善时间,同时对比临床疗效及不良反应状况。结果对照组治疗后有效率(73.17%)低于研究组治疗后有效率(90.24%),差异具有统计学意义(P<0.05);研究组患者临床症状改善时间显著短于对照组,差异具有统计学意义(P<0.05);2组患者均无恶性脏器损害,研究组不良反应率(9.76%)与对照组(12.20%)比较差异无统计学意义。结论两性霉素B联合自体血清注射能显著缓解真菌性角膜溃疡患者的临床症状,效果显著。

Objective To investigate the clinical efficacy and safety of amphoteficin B combined with autologous serum injection in the treatment of fungal corneal ulcer. Methods A total of 82 eases with fungal corneal ulcer of monocular disease were collected and randomly divided into the experimental group and control group with 41 patients in each group. Patients in the control group were treated by amphotericin B therapy; patients in the experimental group were treated by amphoteriein B combined with autologous serum injection. The symptoms improvement time of anterior chamber abscess, corneal ulcer, foreign body sensation, photophobia and lacrimation of the two groups were recorded, and the chnical efficacy and adverse reactions were compared. Results The effective rate of the control group （73.17%） was lower than the experimental group （90. 24% ）, with statistical significance （P 〈 0. 05 ）. The clinical symptoms improvement time of the experimental group were significantly lower than the control group, with statistical significance （P 〈 0. 05 ）. There was no malignant organ damage in two groups, there was no significant difference in adverse reactions between experiment group and control group （9. 76% vs. 12. 20% ）. Conclusion Amphotericin B combined with autologous serum injection could reheve the clinical symptoms of patients with fungal corneal ulcer with obvious effects.
