摘要
目的观察胃癌患者血清miR-200b和miR-200c水平变化特点,探讨其临床意义。方法回顾性分析浙江省舟山医院自2012年1月~2014年6月消化内科及胃肠外科收治的84例胃癌患者为研究对象(观察组),选择30例健康人为对照组。采用qRT-PCR检测所有研究对象血清miR-200b和miR-200c水平,比较2组间血清miR-200b和miR-200c水平差异,探讨胃癌患者血清miR-200b和miR-200c水平与其临床及病理特点之间的关系。结果胃癌组血清miR-200b和miR-200c水平均明显低于对照组,差异均有统计学意义(均P〈0.05)。胃癌患者不同年龄、性别、分化程度和浸润深度之间血清miR-200b和miR-200c水平差异无统计学意义;较高临床分期及合并淋巴结转移的患者血清miR-200b和miR-200c水平明显低于较低临床分期和无淋巴结转移者,差异均有统计学意义(均P〈0.05)。血清miR-200b和miR-200c水平诊断胃癌的AUC分别为0.826(95%CI:0.741~0.911,P〈0.001)和0.856(95%CI:0.778~0.935,P〈0.001)。结论胃癌患者血清miR-200b和miR-200c水平明显降低,并且与临床分期和淋巴结转移密切相关,检测血清miR-200b和miR-200c水平有助于判断胃癌并判断预后。
Objective To obverse the changes of serum levels of miR-200b and miR-200c in patients with gastric cancer and discuss its clinical significance. Methods A total of 84 patients with gastric cancer admitted at the department of gastroenterology and gastrointestinal surgery of Zhoushan Hospital of Zhejiang Province from January 2012 to June 2014 were retrospectively investigated (the observation group) and 30 healthy people were selected as the control group. The levels of serum miR-200b and miR-200c of all people were tested by qRT-PCR and the difference about serum miR- 200b and miR-200c were compared between the two groups and the relation among the serum miR-200b and miR-200c and clinical or pathological features were discussed. Results The levels of serum miR-200b and miR-200c of the gastric cancer group was lower than that of the control group, respectively, there were significant differences( all P 〈 0. 05 ). There were no statistical significances in serum miR-200b and miR-200c with different ages, genders, differentiation degree and depth of invasion. The levels of serum miR-200b and miR-200c of the gastric cancer patients with higher degree of clinical stage and lymphatic metastasis was lower than that of the gastric cancer patients with lower degree of clinical stage and without lymphatic metastasis, respectively, the differences were statistically significant( all P 〈 0. 05 ). The AUC of the levels of serum miR-200b and miR-200c to diagnose gastric cancer were respectively 0. 826 (95% CI 0. 741-0. 911, P 〈 0. 001 ) and 0. 856 (95% CI:0. 778-0. 935, P 〈 0. 001 ). Conclusion The levels of serum miR-200b and miR-200c in patients with gastric cancer decrease obviously and they are closely related to clinical stages and lymph node metastasis; it is helpful to diagnose gastric cancer and pedict prognosis to test the serum miR-200b and miR-200c.
